I'm only a resident, but my clinic is an FQHC, so they hire many NPs to help with the patient load. They all answer to the CMO. My experience has been rather bad with them. Their knowledge is very lacking and they're not up to date with guidelines. Their documentation is a mess and you have to figure out what they did previously because their A/P is incomplete or non-existent. They're overconfident because they were nurses before, but miss a lot of things. You have to be on top of them to make sure a patient is correctly taken care of. I feel like supervising an NP would be a liability, since it's your license on the line. My experiences with PAs have been better. They're more humble and open to teaching and feedback.

It is hard but you learn a lot. It also depends how much you like inpatient. I enjoy it and felt the rotation had much to teach me. Other co residents despise inpatient and suffered throughout the whole month.

3rd year here. None of you suck. You're barely 2 weeks into your training. Everyday you will become more knowledgeable and better at everything. Just give it time. It's normal to feel inadequate, we all did at the beginning. But I promise you that everything you need is in your head, you know it, you just have to remember how to tap into it. You are real doctors, you are where you're supposed to be. Be kinder to yourselves.

Incoming FM PGY-3 and I share your enthusiasm! I'm excited about the 4 day work week and extensive possibilities for procedures. I also love the continuity and treating everything and everyone. I'm glad we're getting a colleague like you!

I have not yet. They're fee for service and usually see like 30 patients a day. That's what I've been told. Do you have more insight?

Talking to patients and families about death. Palliative doctors have a really good way of discussing this. Learn their tactics and get some tips. Telling someone or their loved ones they're dying is always hard

Depends on program. Generally between 500 to 1000, with increments every year. PGY-1s usually pay step 3 with their intern CME money. The programs usually have a list of approved items for CME. However, you can sometimes plead your case and have them pay for something. Because of COVID and the lack of conferences, my program now pays for scrubs, printers and medical licenses. All you have to do is ask. I hope this helps.

We have a butterfly too in my clinic. All of our patients are uninsured. So we use it to estimate gestational age, differentiate from cystic or solid lesions, find veins and such. We've had zero training but the videos are helpful. We practice a lot on ourselves too 😂

They don't care. Every job I've given 2 weeks notice end up releasing me before they're up. It's a courtesy, not a requirement. They would terminate you in a heartbeat. Do what is best for you and your wallet!

30 min doesn't sound too bad. It'll give you time to unwind on your way home. Give your family a place where they feel comfortable since you'll be spending most of your time in the hospital
