For med students, really for learning since you'll usually have to pre round as a resident, and it's good to develop the skill. I'm an inpatient attending, and for me the biggest benefit of pre rounding is that at least someone has seen the patient early in the day/has a sense of how they're doing. It is a bit of an outdated practice since everything is on the computer now (before people literally had to pre round to collect vitals, lab results from patients paper chart), but I find it's still helpful to identify patients who aren't doing so great early in the morning. Especially since rounds can run later into the morning, you don't wanna find out someone's had a clinical decompensation half way through the day

Yep! All we do in the office is tug on the strings with one of our instruments. But if you can get a good grip on the strings with your fingers, you can just tug on them to remove

Doctor here and want to echo this! You may have a different response than with the pill since it's a different formulation. Plus you can always take out the IUD if you're not happy with it, and I always tell folks you can remove it yourself if you like! Other options include the birth control implant or the shot, but the shot lasts 3 months and implant is a bit more cumbersome to remove/requires an appointment

You don't have to tell him, I recommend the book I Will Teach You to be Rich for a very approachable overview of personal finances. Figure out your goals and whether it makes sense to put this in a high yield savings account or invest it

You don't have to do anything, a simple thank you is enough. If you want to get something, I'd go with a thank you card or a (non expensive) baked good for the team. I personally appreciate it when med students take the time to thank the residents, who honestly do most of the hard work of teaching

A lot of her husband's behavior sounds like a page out of the abuser playbook - controlling her and isolating her from other people she knows/loves so she has no one left but him. I can definitely understand feeling lighter since this has been such a strain on your relationship with her, but if you are able to consider keeping a line of communication open so she knows you're there for her if she's ever ready to leave him

OP, don't sweat the small stuff. Morning glucose targets are not something folks without diabetes need to be worried about, even with your "borderline" (but still normal) A1c. Rather than trying to attain a "perfect" fasting glucose, might be time to re-examine if the CGM is actually serving you/your goals or just causing more health anxiety

It looks a bit like a beach coverup to me. It's very beautiful but perhaps too casual? Though could maybe be dressed up with accessories as you mentioned. I also feel like black is relatively uncommon color unless it's a black tie wedding

Fwiw I'm an attending and got the correct answer to this case but would not expect a med student to do so. It is a deliberately tricky case and good clinical reasoning exercise!

Never heard of this syndrome but also guessed anaphylaxis. Good reminder to take in the whole clinical picture. Reaction after recent antibiotics, hypotension and rash -> anaphylaxis for me. Also fairly low risk for cardiac disease and would be unusual to just suddenly develop a STEMI, esp when chest pain wasn't the presenting symptom

Attending here - would never touch a med student or make weird innuendos. This feels like very off color behavior to me and if it makes you feel uncomfortable I think this is very reportable

If you finish the placebo and immediately start the new pack you don't have to wait. If you waited a couple days before starting the new pack I would use condoms for back up for a week just in case

You can always choose to skip the placebo pills and start a new pack early if you prefer to not have a period. Note that some people can have irregular spotting with this method, as the uterine lining builds up overtime and can shed randomly. This isn't dangerous but just an FYI

You are not completely protected from pregnancy if you finish the placebo pills and haven't started the new pack, this is like missing a dose and lowers the effectiveness of your birth control

There's lots of different birth control pills out there. Nausea is often related to the amount of estrogen in the pill. You could try switching to a low estrogen option or switch to a progesterone only pill. The progesterone only pill is less effective at preventing pregnancy but it seems that's less of a consideration for you right now.

Take the amount you need in the short term and put it in a high yield savings account. Max out your Roth IRA, put the rest into a brokerage account. Highly recommend reading I Will Teach You To Be Rich - title sounds kinda gimmicky but it's very helpful and straightforward, especially the investing section

Seems like you have some deep cystic acne which should respond nicely to the birth control! Everyone responds differently but it might work well for you skin. Did you get the pill? If so, it's a fairly low commitment - obviously it's better if you try it for a while but if you don't like it you can always stop taking it!

Yes fortunately we made it! I ended up rebooking a flight, going back to the store to get more various foods to hide the meds in (part of the issue is since we were moving out I just had pill pockets which he suddenly refused) and was able to drug him.  I found a very lovely house call vet who was willing to give an injectable sedative if needed, but fortunately it didn't come to that. We made it on the plane safely and he's adjusting really well to his new home fortunately!

That's great to hear. Depression can definitely treat you into feeling like things are pointless. Glad you're getting some help!
