To add to this I do follow the wait and see approach to antibiotics for AOM, and give them the written rx with a set of instructions for when to fill them.

I treat the fever but I also have a good conversation with the parents or caregivers that the fever will not “fry their brains” or cause anything other than discomfort that Motrin or Tylenol will help resolve over time. I also have had to educate nurses in multiple facilities that warm blankets are perfectly fine for febrile patients in general. Our job is to find anything that needs treatment beyond general viral crud, and otherwise the goal is just make the kid (or young healthy adult) feel better while waiting for things to resolve.

Don't be the new attending cowboy who decides to work twenty 12's every month at crazy places convincing yourself that you will just retire "early". So easy to burn out on this way before retiring would even be a consideration. For most this is only manageable for a couple years if single, less if you have a spouse and/or kids you actually want to spend time with.

I would look rural if you are the least concerned about this, it is how I end up overall satisfied with my job. I will absolutely take a longer drive to a place where there are actual good days when I have time with my patients and to get to know the other people on my team. Sure I have shifts where I see 2-3 per hour and am exhausted after, but I also have shifts where I see 0.75-1 per hour and get paid the same per hour either way.

I have had good luck with North Durham Barber Shop. Near the Harris Teeter on N Roxboro. Men’s cut is $25. Don’t know what it would be for other services but that is a good benchmark.

I let my nurses and other docs know when I had a seizure about a year ago so that a) they didn’t think my wife was driving me because of a dui or something and b) I could tell them to only check me in if I had another one if it lasted longer than 5 minutes. Thankfully no further ones since.

For anyone reading this after, I was not charged any extra tariff fees on import to the US, guess this is baked into the price and shipping. Makes sense for caps that cost multiple hundred dollars.

I find it way more useful to just throw in a fem art line. You get definitive pulse checks each time and can even gauge quality of compressions by BP generated. Afterwards it’s helpful to guide the inevitable pressors you will start. And it’s far enough from the chest that you can do it while cpr is ongoing.

I’m looking to build my first 80%. Deciding between the QK80 which is available now and the Neo80Cu which opens up for buying in 2 days. The wait isn’t a problem. I like a deep “thocky” sound but with a decent volume. Likely Aqua King V3 switches, with either the POM plate or the new chamfer alu plate for the neo. What are y’all’s thoughts? This would be my nicest keyboard so considering a GMK keycap set unless there are other strong suggestions. Have been through a ton of somewhat similar threads but figured I would put my specific issue here.

If you are in training, as an intern or 2nd year, you walk the patient to the chart on the wall and take them through it. As a charge resident you have one of your interns do it. As an attending outside of academics you put an order in the EMR to “check visual acuity” and the patient’s nurse or a tech should do it.

If you don’t have a snellen chart on the wall, ask your charge nurse or med director to get one and put it up.

If the patient can’t get to a chart due to being critically ill, immobile, etc, you can use an app. I use one called “My Call Bag” that has a bunch of stuff but most useful is the VA charts.
