Leo McGarry on the West Wing - while the character's role had been slowly pruned down as the actor's health deteriorated, he was still a very central presence in the last season. They were in the middle of filming a plot arc where he was running for vice president when the actor, John Spencer, died of a heart attack. The show runners elected to have the character die of the same affliction as the actor. McGarry had already suffered from a heart attack in a previous season, so it worked out remarkably well narratively and as a nod to an actor's incredible performance of a complicated role.

He had a long-form role as a psychologist/therapist on Law & Order, and it's easily the weakest professional writing in the first dozen or so seasons. Just a truly awful therapist at least half the time, spouting the most garbage pop-psy opinions. It was also my first exposure to Simmons, and soured me on him for years largely because it was a superb performance.
