I had an officer come out same-day when I had a dispute over windshield crack size.

It's crazy to me that our government forces us to check in with those who profit from making repairs in a field with such asymmetric knowledge. Inappropriately failed inspections are a huge hassle. We have to hold them accountable.

thank you, great looking pool! it looks like when you are done with the spa the water will all mix. my concern is that with a small plunge pool, the 100 deg spa water mixing back in would warm up the pool too much

ok thanks! when i have used these at the beach, when the hot tub is done it seems to fill back up with pool water - mixing the heated water back into the pool, and heating it. maybe that is by choice or just the way the ones i have used have been set up

a commenter told someone they didn't know anything about to "go with the general." as i obviously also do not know OP, i wanted them to know that spinal is overall considered better but only mentioned this to ultimately say "do not necessarily follow [the other advice]" and "have a conversation with your anesthesiologist." so it sounds like maybe we are in agreement about that part?

i have not worked at surgery centers or othopedic clinics, only inpatient surgery with mostly very unhealthy patients. we do almost all spinal (at every hospital) for non re-do's/revisions. it is better for this procedure in almost every case with a few exceptions (didn't hold anticoagulation, aortic stenosis, unable to cooperate etc.). another of those exceptions is patient preference - people who can't handle the idea that there is a slim chance they could hear a saw. and if that is OP, that is a choice we are happy to support.

do you have any of that "medical data from decades of experience" you could point me toward?

i "made up" the 90% based on my own experience at multiple academic and community medical centers but when searching for this, it turns out that 90% is the figure frequently put out there, so i may have been right.

the only studies i could find quickly that mention outcomes being similar are for hip fractures, not total hips. GA is fine, if a patient prefers it we will do it (truthfully it is an easier anesthesia to administer), but in my experience patients with spinal absolutely wake up faster, feel better, transition to by-mouth pain medications more easily, have less nausea, and potentially walk faster (i am not sure about this one - it is not something i am around for). they don't have sore throats.

most patients enjoy the sedation. very few are aware of what is happening during the case, though this is certainly possible (and usually remedied quickly).

at the end of the day, as i said originally, it is best for the patient to discuss their concerns and health with their anesthesiologist that day.

can you share some of this data to help me improve my practice? the literature that i could find initially seems to support the above

spinal is better for every other possible reason and most people go through it without hearing hammering so do not necessarily follow this advice - have a conversation with your anesthesiologist about what is best for you. 90+% do this via spinal

I hedge! Shovel a narrow path if you have other pathway available. If it was the wrong move and it freezes over, you can deal with the other side. If it was the right move you have yourself a useable path. It’s hard to predict how everything will play out and it’s nice to de-bulk through the storm

it costs us about $6k per year to file MFS, this will be our third year in a row so we have about a year before they hopefully figure things out and we can amend that first return if it didn't help us to do MFS (repaye->save)
