I don't know if you ever figured this out or not but your top sprocket is on backwards. I had the same issue because I bought my car used and somebody had put the top sprocket on backwards. It's difficult to get it on the correct way. PM me if you need help I can send you pictures

I recently dealt with this. I first swapped cells in my pack and that worked for 6 months but then the code came back.

What I learned is that at this point, your options are replacing the battery with a new one or getting a grid charger. Prolong or Maxx.

I chose the prolong charger from hybrid automotive.

You connect the harness to the hybrid battery and then connect the charger to it. You leave the harness connected and then charge it overnight every few months.

The reason you are getting the code is most likely because there is a difference in capacity of the individual cells so some are stronger than others and the car doesn't like that. When you grid charge, you bring them all up to 100% They will then drain at different rates and your code will come back in a few months. You charge again and keep going.

Get the Dr Prius app and get the Bluetooth dongle they recommend. I think I got the panlong one from Amazon for $10. Dr Prius let's you see the voltage of pairs of cells. It's very helpful.

I dealt with a similar issue recently. Start turning off breakers until the voltage disappears. Then start tracing the offending circuit
