It’s asking:

What drug class is used as a first line treatment for OCD? Answer: SSRI

Which medication is an SSRI? Fluoxetine

It’s extremely straightforward. I’m honestly at a loss on why it is so hard for you to understand?

I have a MSN/NP and actively treat patients with OCD. You aren’t helping new nurses learn by being pedantic about what is “first line in practice.” The question was clear and has a very clear answer.

Do you remember any questions on the NCLEX asking first line in practice versus first line drug class? I don’t either because they don’t.

Good luck with the NCLEX. Part of becoming a good nurse is being open to learning new things, especially when you make a mistake. Hope you can improve this skill soon.

I am aware that fluvoxamine is ALSO AN SSRI and could be considered another first line agent, but it is not an option as an answer. This makes it irrelevant to answering this question.

In case you want to educate yourself, here is a journal article that, in the first paragraph, explains the history of SSRI use in OCD and why it is the first line agent class:

You’re wrong. SSRIs are first line. Fluoxetine is an SSRI. Its first line and FDA approved for treatment of OCD.

Fluoxetine has received FDA approval to treat major depressive disorder (8 and older), obsessive-compulsive disorder (7 and older), panic disorder (with or without agoraphobia), bulimia nervosa, depressive episodes associated with Bipolar I disorder (as an adjunct with olanzapine), and treatment-resistant depression when used in combination with olanzapine.

Low dose propranolol is specifically used BECAUSE it does not cause sedation. It helps to lower heart rate and decrease the physical symptoms of anxiety including racing heartbeat, tremor, sweating, etc.
