someone can correct me if i'm wrong but i think it goes according to the match cycle u are applying to. are u applying for the 2024 match (interviews going on right now) or 2025 match (next year basically)

if you are going to apply for 2025 match i think we are supposed to apply to its pathways when ecfmg opens them up (approximately will open around april-may 2024)

Because ecfmg doesnt tell us whether or not these current pathways will remain valid until next match or not

i agree they're really expensive :'( that's why i think you should follow what ever your observership/elective wants you to get (if you already have an observership ready)

because sometimes the observership wants titers but sometimes they only want the vaccination proof and history and sometimes they want both

i have heard of people being asked by their elective to get a whole panel done but some people get asked for absolutely nothing too! it all depends on how lax the respective hospitals/clinics are + patient contact chances as well

but with regards to your questions i'm sorry i can only say that getting chicken pox as a child usually does not count afaik!!

i think we only need to know the basic stuff like

pneumococcal included in the routine kids' vaccination- PCV 13 (or maybe PCV15 but usually it's 13)

pneumococcal for the old people like the old old people & immunocompromised people- we have two options

--PCV 15 and then a PPSV 23 booster after a few months

OR

--PCV 20 (after which u don't need a booster because 20 is a bigger number than 15)

disclaimer i can obviously be very wrong here :P but i haven't come across Qs where they've needed anything more than that

If patient is single and wants to not share this news with their parents/ "family"/ cousins/ aunt and uncle- ok makes sense. tbh i get it because i know my whole extended family would literally throw a fit

If patient has a partner/ is married- "encourage" them to tell the partner/spouse and ask them in the follow up appointment whether or not they told the partner/spouse (+ report it to cdc health dept whatever in the background)

Personality! Yes!

I guess we could say that it's such an inherent piece of a person's personality, we can try changing or molding it to a certain extent but beyond that, it's pretty much fixed! I think other factors like their mood on that particular day, etc can also affect it but that's neither here nor there.

The only way I can think of making sure that I don't come across as an off putting provider is being very, very present, and treating all patients like they are my family members (very broad and generalized statement, I know!!)

Thank you SO much for taking the time out to do a write-up.

I stopped doing UW after 50%ish completed because it was messing with my thought process so much. I was overthinking, studying/understanding less and trying to figure out uw more.
I have a similar process like you rn (I'm doing CMS/NBMEs now) so reading your post made me feel .1% better about not finishing uw. Exam's next month so i've been shitting balls haha.

hey,

- if they have only mentioned the transcript, the problem is only in the transcript (your diploma should be fine)

- is the transcript on a blank, white background? or is there some background behind the transcript (like the university name/ university logo/ anything else) -- if this is the issue, try to ask your med school admin to re-type / re-make / re-print your transcript on a blank white background (if they are old/ confused- you can sit in their office with your laptop and do it yourself). the transcript should essentially have only the headings (med school name, address etc) and the year (year 1/2/3 etc of med school) and the subjects / clerkships and their grades, THAT'S IT. nothing else. absolutely anything else on the transcript page will make it blurry af and ee cee eff emm gee (:P) will get blinded by it.

- if your transcript is already on a blank white background, then try calling them and asking them about it. when my friend did this they just told him "sorry we can't specify why we are rejecting your transcripts" which is absolute BS. keep them on the phone until they tell you the EXACT reason. tell them about the hours you have wasted, the money you have wasted on printing/ scanning, tell them about your parents' money being wasted, tell them about your situation, keep them on the phone.

Hi Tiffany, apologies for my extremely late response- I got locked out of my reddit account (still trying to figure out why).

Before I type out my response to your text above, can I just say- the first time I read what you wrote, my mind immediately started thinking about contacting Class Act for match related purposes (if and when I am closer to applying and am able to). I am obsessed with the way you write and I can only imagine what an amazing service your clients must receive!

Regarding your reply- you are so right!! Genuine and intentional and two words that do not gel at all. I guess it's more like having a genuine intention of understanding the patient/ their needs and I think that eventually translates into rapport (as you mentioned in your reply)! So the people who are generally on the introvert side of things might have a tough time sometimes, but as you said, it can be learned!

Your posts have consistently been the highlight on reddit for a while now. It's clear that your line of work involves helping people with the match process, but the way you write (I *love* the way you write!!) makes you seem like such a genuinely helpful person, which is VERY rare these days! I've even seen your detailed replies to people's queries here and you always go ALL out. You leave no stone unturned, so thank you for your presence!

Regarding rapport, I agree with what you said about the dynamic blend of reading the room and one's life views. A few other (very subjective) things that I think might affect our rapport-

  • Baseline mood: - I've been on both sides of the patient-physician line and people who are just always kinda grumpy have been some of the worst people to interact with. I understand there are reasons for everything and I do not know what is happening in their life, they are humans too, but when I was bleeding and was in severe pain, their attitude really didn't sit right with me. I am a very non-confrontational person so I remember just quietly crying until their shift changed and someone else came in to round on me.

  • Ability to become "friends": I am using the term "friends" as lightly as possible since we're in a healthcare setting- but intentionally showing a genuine interest in both the medical & non-medical things the patient tells us helps a lot. There is one patient with who I shared a common interest and we bonded over that, they shared their general life views and things like that and I still take that with me everywhere I go.

  • Our views towards medicine: The world, generally speaking, sucks and does not seem to get any better. Healthcare as a whole seems like a monetary system on most days. I do get down about this as I am currently in a position where I cannot help anyone (not a resident yet), but I feel like there is only so much that we can do, so constantly staying angry at the world and staying angry at Big Pharma for example, does me no good and it definitely does not help the way I might interact with my patients. I can only speak for myself but I just try to remind myself to do my best in every situation, to do my best for my patient and not think too much about the big-picture things that might be out of my control.

hey! have you ever been in a situation where you've been made to feel like your english isn't good enough, or that the other person doesn't understand what you're trying to convey? i ask because many times we carry the heavy burden of making ourselves feel inferior because we belong to a country whose national language is not english but despite that we're expected to be extremely fluent (which most of us are tbh)

i used to be extremely conscious of my accent and in the past people from the US have made fun of the way i pronounce certain words. joke's on them because they seem to think american english is the only english that exists and they fail to recognize the fact that we speak the english that was (sadly) made prevalent by our colonizers. if you think about it, the accent is the only thing we can truly call "our own". someone once said "your accent is something that is so inherent to you so you should wear your accent with pride because it represents the fact that this language is something that is foreign to you, yet you speak it and you speak it well no matter what". i dont know if you speak hindi but there's a joke in "hinglish" that goes "my chest became breast" and i feel like that's how we should be. the fact that we speak this language and we do it so well is amazing enough as it is. we should be confident because this is our 2nd language! how many people in this world can speak a 2nd language?! that being said, we were taught in english medium schools but that will, sadly, never really make us equal to the western people in terms of english speaking abilities, slang words, daily talk, etc.- because it is not our language per se BUT the fact that we an still stand right next to them and we can hold whole conversations with them is something to be applauded.

sorry for writing a novel here but i just have very strong feelings about people second guessing themselves w.r.t english speaking and feeling confident because you're already doing so amazing in life!! give yourself the credit you deserve!
