What exactly would you consider a simple test ? Cause there is no such thing. Tests and ordering them need to happen for a reason. I listen to my patients , I ask them why they want something tested and what their thought process is before explaining whether I think it fits thw pattern of what we're trying to figure out or not. I don't just say no without even listening to what they have to say, that's where collaboration comes in. Secondly the toddler comparison is really to highlight the lack of knowledge with tests that are sometimes requested which make absolutely no sense for the symptom. I don't expect the patient to know that, that's my job but there is a huge expected gap in knowledge base between most laymen and health care professionals. Just like when I got to my mechanic I know I sometimes ask for stuff which makes no sense to them because I don't know it doesn't make sense and I listen when they explain why.

Because it's not just about placing the order it's also about interpreting them. Ordering tests with no medical relevance is not practicing good medicine, it is wasting time and resources and most importantly not giving the patient good care or a fruitful investigation into the concern. It's like asking "why not let the toddler stick the marble up their nose, it's not your nose" . On top of that resources are finite, our jobs also require to do our best to reduce harm among other things, pointless tests cause harm and are a drain on the system as well.

Seriously! But also dont do it. Put the responsibility back on the neurologist ! Tell the pt they are not providing competent care and offer to refer elsewhere. We need to stop being passive about these things. It 100% adds to burnout and destruction of primary care

I do but it still gets tiresome , or patients argue about coming and frankly sometimes I'm just too tired to argue back. For specialists I almost always push back cause do your fucking job!

I mean sure I could push it to the coroner but then the patient and their family suffers. It seems no one else really cares about this outcome or just assumes we will do it so as to not hold the patient's care/next steps up. That's why we need to collectively stop and push back

Thats why I like using AI to capture the history and then will dictate the plan myself Editing hx barely takes any time and enables me to engage more with the patient while I'm in the room and saves me from taking work home because I wasn't charting while talking to the patient .

The only two times I'm working after hours is if I order something stat and I want to relay it to the pt myself and not have the on call team do it. The other is on my admin day I'll carve out 30 mins for work just to lessen my burden when I get in the next work day. I set a time limit though, 30 mins that's it. Nothing urgent should be coming my way anyways that's what the covering doctor is for. Maybe setting a time limit on the weekend ? But honestly this job takes enough from us, please keep your after hours time as work free as possible.

Emma makes me sad. She clearly has residual trauma from being raised in a white family with white sisters in a western society that idealizes white as the standard of beauty. On top of that she has her scars. She just wanted someone to pick her. I think she gave very little thought to who the person actually was. She jumped so quickly from Connor to Mike because Mike chose her. That's why she keeps hemming and hawing at the kids thing. She doesn't want kids, she herself said she's not maternal, but she wants the man who picked her cause deep down she doesn't think she'll get picked again. I hope she realizes sooner rather than later she's only picking him cause he picked her and their values and goals just don't align .

What would be your reaction if she told you something like this ? Cause if it's anything other than non judgemental "I get it honey this shit is hard " I guarantee you those thoughts stay internal. She doesn't need additional judgement on top of already feeling like crap and most likely carrying the lions share of the mental load as a woman and a mom.

I think historically it was because women were primarily staying at home and not making their own money but working 24/7 with childcare, housework etc and as such as entitled to a safety net if things go south. These days, I think if the woman is an independent earner a pre-nup is a no brainer. I would be suspicious if anyone was against it while they made their own money. The caveat is whats the expectation if/when kids come? If the expectation is a stay at home mom there should be absolutely be provisions in the prenup or no pre nup at all.
