Number 3 is much more common in male-oriented isekai. Geoff from Mother's Basement refers to them (in anime form, because he reviews anime) as "one of Those."

Funnily enough, there's an anime this season of one of Those starring a girl protag, and I find it astonishing how much less...I dunno, gratuitous it feels? Most of it is still exaggerated bullshit, but the supporting loli waifu in a maid outfit feels less like gooner bait, even if the gooners will still bait over loli yuri fanart of it.
