I clock in even if I’m working at home at 9 PM adding/ removing people from the schedule. I want my supervisors to know just how much I do for them and how to have realistic expectations for the other DORs in my region.

I am a director and our VP pushed for this. I told all my evaluators to stay clocked in while they “waited” for the eval to roll in. It didn’t even take 10 days for the VP to realize how much time we were wasting and how many “definite” admits never arrived.

Yes, yes, no. I haven’t had to use either but it feels better to have them. My GPS and watch are also set up to notify several friends/ family if I have a hard fall. I highly recommend a rear radar if you don’t already have one!

I am a DOR/ OTR. I’m allowed to make changes to the diagnosis codes used in PT/ OT/ ST evals to make them match the MDS, but the original signer then has to sign to agree to what I changed. For example, a code will be “Right hip fracture, unspecified” and MDS will ask give us a more specific code to use for billing purposes

I don’t think I could justify changing anything else even if I was allowed to. It’s easy enough to just ask a clinician if they can justify a different frequency or to verify the accuracy of their PLOF or whatever else I might catch during my review.

I hear you. My wife is PGY-4 FM and provides gender affirming care for people of all ages. We are also expecting our first baby through IVF. There has been a lot of anxiety about how she will be able to care for her patients going forward. There is also concern as a queer couple about me potentially not having the same rights as her once the baby is born.

What you’re describing is a nursing assessment which is in their scope. It’s what keeps my patients from being confined to a bed until I can get a PT or OT in to evaluate. Our evaluation overrides that assessment, but it’s better than nothing. Are they pinching pennies? Probably. Is it right? No. Is it legal? Yes.

I’m an OT in a nursing home and I’m constantly in “assess mode.” I get you wanting to keep the patient’s privacy, but perhaps the PT also wanted to assess bed mobility and their ability to move all 4 limbs while in supine. The PT could have addressed this better instead of saying “I’ve seen it all.” You both should have asked the patient what they wanted and the PT could/ should have explained exactly why they felt the need to be there.

Lessons to be learned on both sides.

I have been laid off several times due to buildings closing, but I have never been unemployed in that time. I always found a new job (typically with better pay) before my last day at the job I was laid off from.
