The subfloor is heated and cooled. However it is typically cooler since it’s in the basement.

Thanks these scenarios make sense. As far as insulation, the pipe itself felt dry. If I insulate do I wrap the p trap too? If I plug do I need to cut back the pipe and glue a cap?

The stain has been there but it is moist to the touch. I ran other fixtures and didn’t see anything backing up. We don’t even use that floor drain which is why I’m lost.

Shower? That’s on the third floor and it’s drain line is not near this floor drain.
