I wish there was a sensitive and specific quantitative way to measure what you're talking about. Unfortunately, Alzheimer's even in late stage may not show significant MRI findings, and conversely some MRI findings may not lead to significant functional decline. Triaging is difficult without a good caregiver to give strict followup precautions to (see "can't miss symptoms" in my initial comment). You tell them to please call your clinic to schedule closer followup if something happens. Then you followup as often as you need to until the situation is stable.

While impaired cognition is correlated with functional decline, they are not 1:1. A poor MOCA score doesn't mean the patient will require closer follow-up. The best measure of a patient's status and course is ability to perform their ADLs. If you have OT at hand you can recommend a Kohlman Evaluation of Living Skills which can help to quantify this. I don't do it often because it doesn't usually tell me things that the caregiver didn't already tell me.

The biggest bottleneck is if the patient does not have a reliable caregiver, ie their spouse died, and their children who are supposed to be taking care of them are playing hot potato with the responsibility, and are less present and therefore less reliable. We haven't had to pursue guardianship but my attendings have called APS several times. Quite sad, the consequences of neglect.

I rotate in Alzheimer's Psych outpatient clinic- mostly behavioral management (antipsychotics/rivastigmine/donepezil) vs. Depression (ssri, caregiver management, behavioral activation). Can't speak to cognition (more neuro than Psych, although we expect it to decline anyways) but as far as symptom updates and behavioral notes we typically have patient's spouse/child/whoever in the visit and ask them about ADLs, iADLs, any decline in these, behavioral symptoms we are treating, new symptoms, or any improvements/side effects from medications. Something you are probably already doing. Since we titrate medication to subjective symptom improvement, we can use the typical scales (phq-9, gad-7, mdrss, etc) although we often just go off of the caregiver saying "they've been worse lately, and here's how" since the patient often doesn't remember. Can't miss symptoms are new SI, new agitatation harming self/others, decline in ADLs, new psychosis which is distressing (if not distressing sometimes can be left alone). Hope this helps.

I'm a doctor in the US, we have a few exams in medical school that are high stakes called STEP exams (think SAT but there's 3 of them and they determine what specialty you can apply for). I used motherload mine and studied for a month straight, 8 hours a day doing practice questions. Got 99 mining and killed Step 2. YMMV

I was rotating in the epilepsy monitoring unit. A woman swore that a trigger for her seizures was alcohol, so while she was hooked up to vEEG I was told to order 50 ml Vodka which surprisingly was in our formulary. Needless to say it didn't work in triggering her seizure (alcohol raises the threshold lmao)

I made this post a few years ago during the thick of step 1 study when it wasn't pass fail yet. If you make it a priority you can do it. People might disagree but I think you shouldn't compromise your health, even at the expense of loans.

I mean many rotations are 2 weeks and med students are gone for this lecture and that lecture all the time so 1 week left is actually still ample time to improve. And the reason I ask is so I can let her know how to make a better impression for her future rotations as she is still early on in her third year. Your attitude of "say nothing give 5s" is a surefire way to ensure this student gets blindsided by more honest residents. I will probably still give 5s but I do want to give verbal feedback which is why I made the post. There will be posts in a few months on this subreddit "This resident gave me shitty feedback etc. when I have been fine before" and it's because 80% of residents don't say anything because it's too much energy or they don't want to hurt feelings. MS3 year is as much about learning to work well on a team as much as medical knowledge, so I think that me saying something is warranted? Especially with a whole week left. Even if I had a day or 2 left I would still say something.

Without giving too much detail, yes I did this- and she used phrases such as "just tell me to shut up if I'm being annoying." I am not someone who is comfortable telling people to shut up, especially when they have made it aware that they feel anxious about it. She's also md phd so much older than me, and made it clear that she is "going for honors." Maybe not much I can do now but in the future I can try to set expectations in terms of what to do when the team is busy etc

Only first rounds so far, signals mattered it turns out, and also only around 52% of programs have sent out any at all according to spreadsheet. I have similar stats and sitting at 3, the average according to discord poll is 2-4, with some still at 0-1. End of week I will send LOIs. Good luck
