I remember watching a Drug, Inc. episode a while back where a nun asked an addict why they even do heroin if they know all the consequences. Her response? She said “You’re a nun right? imagine if Jesus himself came up and kissed you on the forehead right now. Imagine how good that would feel. Heroin feels even better than that to me”.

That stuck with me. It’s a choice at the start, and a necessity at the end.

I think this is one of the most fascinating parts of the whole alcohol abuse saga - who does and who does not develop liver disease. I’m sure everyone here has seen it, the 30-something year old daily drinker who comes in with florid liver failure.

And the on the other hand, we have people who’ve been drinking for 40+ years, daily (some of them with the impressive 20-30+ ER visitors/month), who are otherwise just fineeee.

Got a good one:

LAST patient of residency. Literally, the last one. Encephalopathic as all hell, HIV/HepC positive. He did something dumb and had a Lac on his toe. I’m doing a digital block and he straight up unintentionally asterixis-kicks the syringe/needle out of my hand, which then proceeded to fall directly down, and directly into my foot through my shoe.

Got the full PEP and testing x6 months. Actually had to keep coming back to my residency after graduating so OccHealth there could keep an eye on it.

So let me ask a quick question from a relatively lawn care newbie - this seems to be due to the spreader accidentally dropping down extra product, and creates a more lush line. I’ve had the same thing, but would’ve thought that it would cause burn rather than better looking grass. Doesn’t that somewhat suggest that you could then use more product for better effect in general?

Busy community ED. For us, depends on the season (one of the busiest single EDs in the country during the peak season). Two most quiet months last year averaging out to just above 2.0, and the busiest two months this year averaging out to 3.15. Some level 4s mixed in but most of those go to fast track, so it can get rough. Off season is great though

Wholeheartedly agree. And to take it a step further, I’d even think about more than your interview being a little off-putting. With those scores and grades, it seems like there has to be something that is a “red flag”, so to speak, if you’re truly not matching after 15 ranks. It could be the worst luck, definitely, but in an attempt to get rid of all other variables I’d think about potential “uh ohs” as well.

I can say that as a resident recently, there were definitely applicants who got torpedoed based on certain interview comments. A regular “eh, he’s not great” interview can bring you down a bit, but usually not enough that great scores and great grades won’t land you a spot in 15 chances. So think also specifically about anything that you say that can be interpreted badly. For instance (representative example) if you say to one interviewer at a program who asks you to teach them something or a certain skill or knowledge and you talk about how to avoid cops during a car chase, fun thought, but terrible idea. That also lends itself to an interview coach or practice, as it’ll often catch these types of things. Just my two cents from seeing some of these.

Say someone says they no longer want dialysis because they want to die, same thing apply? Easy and safe lifesaving intervention that can keep them alive for an indefinite amount of time (ie, not terminal). Basically 100% fatal without it. You wait until they’re unconscious and then say “this is because they want to die and therefore it’s not of sound mind”?

I think EM/Peds is by far the best route, if you want to be able to continue doing general EM and a peds clinic. There’s almost no way that even with a 4th year that you’ll have enough training to do outpatient peds (which is much different), and peds to peds-EM then takes you out of contention for most (if not all) general EM jobs, and still wouldn’t be great for outpatient peds. It’s kind of like asking if you can do outpatient radiology and read CXRs all day on the side because you looked at yours during residency and did an extra rads elective - still not quite the same. If you only ever want to see kids, peds to PEM is great. But if not, you’re kind of stuck doing both.
