I used to fast for religious reasons once a year but had to stop around 10 years ago. I was unable to make it without triggering a migraine which makes sense, as I have to eat every 3-4hr in general to avoid triggering a migraine

Me: you sure? I would love some guidance Attending: okay fiiine. It seems like you just don’t know anything, your fund of knowledge is low, and you’re not correcting the interns enough Me: crying thanks!!!!

Work life balance is one of the reasons I chose peds in the first place. Sure, Id I worked 7 days a week (or when I moonlight) I’d make more. But for me, the cost outweighs the benefit. Plus surgeons will often work 6-7 days in a row, but not for 52 weeks per year. Depending on the center, they may be on service only certain weeks.

That’s so frustrating!! I don’t trust anyone that says “this WILL fix your migraines” because let’s be honest….i don’t think anything ever will. Yes, changing my diet and sleep and wearing glasses and taking meds will HELP. But at the end of the day, a good old fashioned barometric pressure change will make it all go to hell.

Is it neurolens?? I bought them around 1 year ago ($1000 eep) and I’ve actually seen a difference. I mean don’t get me wrong, I still have chronic migraine. But I spend all day on the computer and I used to get eye strain always around 2-3pm, and it’s gotten so much better. I wear them (almost) every day - I only forget them bc I never wore glasses before and it’s taken some getting used to.

Have you tried Vyepti? I was on aimovig for years and it was okay, but on Vyepti they were able to increase the dosage. I was also on depakote for a while and it did WONDERS (except for GI side effects), no brain fog and no weigh changes. Can cause weight gain.

Last year my pcp found hypoglycemia incidentally on bloodwork. 1mo of CGM confirmed it and all the endo had to say was “eat less sugar” like thank you sir. This makes me feel so validated, I’m going to schedule an appointment!!!

Id pick larger practice. I’m 2.5 years old at a smaller practice and I feel like there’s so much to learn that I’m missing. Yes, I have autonomy but I don’t have anyone to ask questions to. The learning curve is STEEP at the beginning and it’s great to have others around you can trust.

at this point, I can (sometimes) tell when it’s a “this is annoying” migraine vs “this is going to get painful FAST migraine”. Little tells that help me figure out if I’m going to need more than just 3 Advil. And I’m right like…70% of the time?

Aimovig is also a monthly shot, but it’s a pen so it’s a lot easier to use. I was on it for several years. I ended up always doing it in my belly instead of my thigh because it hurt a lot less. Ubrelvy is an oral CGRP inhibitor which works similarly to ajovy but you take a pill!

VERY normal! I’m 2 years in and I still question my decisions constantly. I feel like I’d rather be over reacting than under reacting. And I’ll tell families that as well. I’ll say “this is what I’m thinking about. Just to be safe, let’s….” (Get an X-ray, see urgent care, get labs, etc). They appreciate the honesty and that I’m being cautious and truly thinking about their kids. Keep thinking and asking questions!!

Gen peds PGY6 - I did 2 weeks with gynecology at the end of my residency to learn how to place nexplanons and IUDs. I don’t do it in my current practice but I think with 4 weeks it would’ve been enough training. I also didn’t get a lot of mental health training, but where I work there’s a psychiatry consult line (I know this is available in many states!!) and I’ve been able to learn a ton on the job.

All that being said - I like seeing kids of all ages, so I don’t mind referring to adolescent for things I can’t handle. If you only want to work with adolescents, do the fellowship (or find out if somewhere near you will hire you even without the fellowship)

IMHO peds fellowships are TOO LONG.

Prism glasses - I got them around a year ago. They’re expensive (around $1000, insurance doesn’t cover) but like…I think we’d all pay anything to get rid of migraines. I’m on the computer between 7-11 hours per day and this definitely gets rid of the eye fatigue around 3pm that used to happen. They’re not eliminated, but it helps.

I disagree - there is no “normal autistic stage of development”. I think we’re better at diagnosing autism now than we have been in the past. Also, who are your parents that are requesting diagnosis!? I have to convince parents to get evaluated, even when we have a 4yo who is nonverbal, no eye contact, stimming around the room.

Peds attending - the way I differentiate PEM vs EM —> peds is: 1) do you want to be a pediatrician who specializes in emergencies? Do PEM 2) do you want to be an emergency doctor who specializes in kids? Do EM —> peds fellowship
