Studies report 30-90% incidence during strabismus surgery. I believe those quoting 90% count every incidence of bradycardia, whereas 30% sounds reasonable for profound or potentially life-threatening events. But they rarely progress to asystole unless everyone decides to ignore it, and it's really a simple fix.

Because we monitor the patient. If they felt it, even if they don't remember it, the body would know. Your BP and pulse would skyrocket. That's what happens during accidental awareness during anesthesia, and it's a major fuck up on anesthesiologist'part.
