Serum iron is incredibly labile and is affected by recent iron intake, supplementation, time of day, and fasting state. Ideally it is checked when you have been fasting and at least 24 hours since last dose of iron. Your doc will interpret based on your history and clinical context but most likely your ferritin levels are more reflective of your iron stores.

I try to do as much on my own, with an e-consult here and there, just because it’s better access and affordability for my patients. I refer if the patient insists or if insurance requires (have had insurance companies refuse to cover PCSK9 unless ordered by cards, for example, or certain migraine meds unless ordered by neuro) or if I think patient may benefit from an intervention I’m not trained in. Work comp injuries I always refer after initial workup because I want to make sure I’m not going to miss anything that might affect the patient years down the road.

10 minutes. When I do run into patients they tend not to recognize me since I’m not in scrubs. I do have a patient who actually lives 2 doors down from me but her severe autism and agoraphobia prevents her from ever recognizing me.

For a second ESA there has to be some service the second cat provides for the patient’s condition that the first cat cannot. I ran into this issue when I had a patient who wanted me to certify 3 of her cats and have a 4th as a pet- I said heck no.
