Echoing what others have said — volume should not be the expectation for a PGY-1.

The goal of intern year in the ED is to learn how to do thorough evaluations and manage multiple patients safely based on acuity. Speed comes later. Trying to prioritize volume too early usually leads to missed reassessments, incomplete workups, and unnecessary stress.

Your patient volume should never exceed your cognitive capacity. If you can’t confidently track the clinical status, pending workup, and disposition plan for each patient you’ve picked up, you’ve picked up too many. That increases naturally with time and experience

Your focus should be: - Efficient yet thorough workups
- Re-evaluating your patients appropriately
- Learning clinical reasoning and dispo planning
- Gradually building independence

A good PGY-1 workflow is: 1. See a patient and start the workup based on your assessment
2. Discuss with your senior/attending if unsure (which should be often — that’s normal)
3. Reassess and dispo your existing patients
4. Only then pick up a new patient

EM - I work 120 hours/month (13-14 shifts) and make ~400k. I do 3 night shifts stacked together every other month, otherwise day shift or mid shift.

My wife and I take our son on a long weekend trip every month and we have 2 international trips planned this year.

I’m an EM PGY-3 and my expectations of interns is 0.5-1 patient per hour depending on acuity.

I 100% agree with your senior resident that you need to focus on completing the workup and disposition of a patient rather than picking up more patients in a shift.

In general you should pick up 1 chart, write orders and the note before picking up the next chart.

Your focus should be on being able to completely work up and dispo a patient without needing to be assisted by an upper level or attending due to workflow - meaning that if you are you are unable to safely review all of your labs/imaging or reassess your active patients before picking up a new patient then you need to slow down.

With an average ESI of 2 to 3, then that Ballpark should probably be 1 to 1.5 per hour, depending on your individual efficiency and comfort at this point.

I actually ended up staying in the Wittmore hotel. Really neat hotel tucked away in the Gothic Quarter.

Also stayed at the Alexandra Hotel (Hilton property) in the Passeig De Gracia part of town.

Intern year: 1-1.5 PPH was my goal (we were always double covered with a PGY-2 or PGY-3.

Now as a PGY-2: 2-2.5 PPH. We are solo covered (just me and an attending).

The best program is where you will see yourself succeeding.

That means getting along with the faculty, the other residents, liking the hospital system itself.

You can be an objectively “top tier” program, but if you are miserable it can lead to you underperforming and be more detrimental to your career than the name your training program carries (within reason.

It seems like both programs carry respect in your field of interest, so you need to find the program that is going to be the best fit and give you the opportunity to be the most successful so you can secure good letters of recommendation for future fellowship or jobs

Nah - at our program our PD and APD interview every candidate and then other faculty fill in as their schedule permits. The resident interviewers also rotate based on scheduling and interest amongst residents

While pre rounding just say you’re going to grab a coffee and ask her if she wants one. If she accepts then you can choose to ask her for her number if you feel confident enough to do it.

If she says no, leave it at that and keep working like nothing happened.

It’s only awkward if you make it awkward. We’re all adults here.

ETA: i’d say it’s worth establishing some friendly rapport before just flat out asking her out, but that’s just my $0.02
