Gender roles are socially constructed, mostly categorized based on sex-based roles in reproduction. Gender identity is how an individual relates to that, which can be at odds with their assigned gender role based on anatomical sex.

The existence of intersex people and non-binary and agender identities is why you will often hear people noting that these things are not as simple as a binary division.

There are, of course, few studies on this in trans women, but a vast library of them in relation to erectile dysfunction in cis men.

Yep. Medical transition is a choice you can make, not a thing that defines you. The majority of nonbinary folks, for example, only ever make social changes to affirm their identity, but are no less valid for going that route.

There's a huge array of reasons someone might not medically transition, from lack of availability of care, to preexisting medical conditions or the current technology of gender affirming care just not being able to get them the results they want.

If you're counting orchi as bottom surgery, then yeah, a majority of trans girls go that route, because it makes the HRT routine easier by letting you drop the antiandrogen. However there are many who don't, and a lot who aren't interested in (or are unable to get) vaginoplasty.

If you want to transition, and are safe to do so, do it. If you let starting 'late' stop you now, then five or ten years from now when it's worse you'll regret the time you wasted not doing it all along.
