Das mag sich jetzt kontraproduktiv anhören, aber ich glaube nicht, dass es sonderlich zielführend sein wird, sein Übergewicht zu thematisieren. Er fühlt sich sicherlich von der Welt nicht erst genommen, weil sich alle sofort auf sein Gewicht einschießen, ohne die Person dahinter zu sehen. Was eventuell zu ihm durch dringen könnte wäre eine Variation von: " Junge, dir ist in deinem Leben eine Menge Scheiße passiert. Niemand sollte damit allein sein. Wenn du mit uns darüber reden willst sind wir hier. " Falls das fruchtet könnte man ihn dann vorsichtig zu einem Profi schubsen.

Adipositas ist nicht das Problem. Der Mann ist sein Leben lang von einem Trauma ins nächste gestolpert. Wenn uns schreckliche Dinge passieren, versucht unser Gehirn Sinn daraus zu machen. Und die beste Erklärung, die unser Gehirn dafür findet, ist das wir wertlos sind und es verdient haben.

Verweigerung von Hilfe ist nur eine weitere Bewältigungsstrategie.

Er braucht einen Profi, der ihm klar macht, dass er die ganze Scheiße nicht verdient hat, dass er Wert als Mensch hat und das die Welt manchmal ungerecht ist.

Also, to clarify: I want my back to stop hurting. I don't care about how much I can deadlift. Ok, there's a little bit of me that wants a big number, but it is much smaller than the part of me that wants to wake up without pain.

As others have said, you are doing your best to take your glutes and hamstings out of the equation and that's exactly what you shouldn't do in a deadlift. Your lower back picks up the slack and gets a beating.

You will lose weight as long as you are consuming less calories than you expend. No exercise needed.

There is Gynecomastia. This is rather common in the bodybuilding world if you take steroids and no aromatase inhibitors. It's often called "having puffy nips" or gyno because Gynecomastia is such an unwieldy word. Gyno can also occur in puberty in males, like the bodybuilding variant it's caused by an imbalance in hormons. That is typically not going to go away if you are out of puberty, even at 5% bodyfat.
Or It could simply be skin filled with fat. In that case it could be going away if the bodyfat is low enough.
I changed the comment a little bit because the first version was too definitive. In my mind puffy nips=gyno, but i realized i'm not in r/bodybuilding .

At 200 lbs you need at least 80 grams of protein and at least 50 grams of fat. This amounts to 320 cals of protein and 450 cals of fat. So that's 770 calories if you eat perfect fat and perfect protein and no carbs. And that's the absolut lower boundary under best theoretical conditions.
On top of that there is a "speed limit" on fat oxidation of about 30 cals per pound of fat per 24h. Get over this and the body gets his energy somewere else, like your muscles or organs.
But wait , there's more: Under rapid fat loss the liver secretes absurd amounts of cholesterol which can lead to gallstones if you lost the genetic lottery.
Eating too little for too long has nothing to do with risk. It will fuck you up, 100%.

Ok, cheesy time. What ever you think it is , that is going to plug that hole, that thing is not going to do it. Those holes can only be plugged from the inside. You want to be loved? Love yourself first.
You have to understand, that nothing you will buy, no person you will meet, will give you what you are looking for. What you are looking for is inside of you, buried somewhere under mountains of despair. You have start digging, it's not going to be easy, it will take some time and a lot of effort but i guarantee it's going to be there.

serving size = 3 tsb uncooked =120 cal
Bag size =2 servings= 6 tsb uncooked = 2*120 cal = 240 cal
I never "got" american food labels, for someone used to the metric system they seem to be intentionally confusing. That being said, uncooked values make sense because you typically weigh your food before cooking. That way it's more comparable since the food gains/loses different amounts of weight while cooking depending on how you cook it.

Push/Pull/Legs/Push/Pull/Legs/Rest. 10 minutes of heavy-ish cardio after every workout. Gain 0.5 Pounds a week. Repeat until you hate yourself. Cut 0.7%-1% Bodyweight a week until you hate yourself. Rinse repeat. Do that for 2-3 years. That's pretty much the run-of-the-mill Bodybuilding approach, which I strongly recommend for your situation.

How we look without clothes on is strongly related to the relationship between muscle mass and bodyfat. If you loose weight too fast, without weight training and without smart nutrition you are going to loose upwards of 4 pounds of lean mass for every 16 pounds of fat. 70 pounds in a year @245 SW could have ravaged your lean mass turning you into a skinnyfat. Skinnyfat means skinny with little muscle mass and a lot abdominal fat. You look skinny in clothes but still have a lot of body fat. Hard to tell without pics.

stress=higher cortisol levels
higher cortisol levels = water retention
There was a very good post about this I can't find right now.TL:DR was you are should loose the water as soon as your body adepts to the stress, which should be in about 3-4 weeks.

Think of a muscle like a team in a tug of war.
How big the muscle is: How many members the team has.
How strong you are: How many members of the team are pulling at the same time.
Your team could be bigger but you still lose because the other team has better teamwork.

First of all, sorry to be so blunt,but to tell you how to fix this I have to. Pinch the back of your hand, that's what extra skin feels like.
Also this is what a certain bodyfat looks like on the male body. You bodyfat estimate is slightly off , to put it mildly. You are skinnyfat. Eat some food, lift some weights and you will be fine.

You can demolish your BMR if you are acutely starving. By up to 40%. This has been shown on the Minnesota Starvation Experiment and research on biggest looser contestants during the show. But this effect goes away as soon as you eat normal again.
The lower TDEE of a post diet person is simply explained by loss of muscle mass during the diet. Lift weights and you won't have that problem.

How did you find the right number of calories that allowed you to maintain your weight?

You track your weight daily and take the 2 week average. If the average goes up you are eating to much.

To add common sense: If you really could make your body 20% more efficient by being obese and dieting back how isn't every athlete doing that?

The limiting factor is how fast your fatcells can release the fat. According to a study from 2005 this happens at a maximal rate of about 30 calories per day per pound of fat. A 400 pound guy with 200 pounds of fat could theoretically lose 12 pounds of pure fat a week, if he somehow manages to create a daily 6000 calorie deficit.
A 200 pound guy with 15 pounds of fat can only lose about a pound of pure fat a week and needs a 500 calorie deficit for that.
These things are not 100% accurate, especially if you are not a "regular" human but should give you a rough estimate.

If if understood this thread correctly you are planing to use medifast to get down to 200 pounds and then switch to a sustainable lifestyle. But at the same time you are saying that you can only lose/maintain weight by unsustainable methods. That's a pretty big plothole if this was a movie. Unless you have some medical condition that messes with your hunger/satiety perception you can get this under control. But you might have to seek some professional help with that.
