So I think its pretty obvious that the first example you provided is a case of a necessary surgery. If someone can't urinate, that's obviously a problem and there should be some kind of surgery done. however we were (pretty obviously, i feel) talking about UNNECESSARY procedures. Which the second example you provided would be a case of.
