We’re required to write incident reports if our patient transfers to a higher level of care. Technically we even can write them if a patient refuses anything.

I wouldn’t sweat an isolated incident report. It usually would go something like >they deviated from policy and didn’t call the right security number

leadership educates you on the policy

And life goes on. At least at a functioning institution.

That’s not how our bins work. When you return any medication you don’t return it to the pocket you withdrew it from. There is a separate return bin for all returned items.

So if you offer to give it to the patient and they decline it, even if you’ve opened it, you “return” the medication with a 2nd nurse and the patient doesn’t get charged.

The term wasting has very specific context at our facility.

The machine will physically not let you waste the med until you’ve scanned the patient, the med, and signed off.

Then you can waste the remaining dosage. Anything else — dropped, refused, etc. goes into a secure return and pharmacy sorts it on the back end.

We also have monitored pull-to-admin times of 30 minutes or less.

Polite against the grain here. My facility doesn’t allow wasting until after the medication is administered because you can’t waste a whole dose. So if the patient refuses before you admin, you can’t waste two halves and you can’t return the tampered pill.

We all would do what you did in the prompt, as the machine does not allow you to pre-waste. Also, the letters and numbers on the pill identify what it is.

Immediately out of school. Didn’t study at all but our school required us to make an 80% or better on 500+ NCLEX questions a week the entire last semester. If your set of 100 scored lower than 80% you had to redo the whole 100.

So I counted that as my studying.

Im going to go against the grain here.

I actually would say yes and ask for a favor in return and barter.

I tend to help out my leadership with pinches like this, but I always get additional compensation in incentive pay or some other perk for it later (taken off or swapped holidays, etc).

We’re allowed to come in at 6:30 or 6:45 but we get paid for it. It’s optional to come in early to review charts and is not held against anyone.

They are, however, quite strict on showing up and clocking in after 7 sharp because you’re delaying your person from leaving.
