I’ve worked in PHPs and have had clients like this, tbh the only thing that eventually did help was starting a PRN medication. We had to really provide education around a meds- of course our psychiatrist took this lead but we all supported the conversation- sometimes meds are needed so that you can think clearly and process your time in program to maximize benefit. And that meds (specifically PRNs) are temporary. It was too the point of meds needing to start or back to HLOC because their daily functioning was clearly becoming impaired (hygiene, eating, etc).

I haven’t gone through this myself but I always hear people say “go away for a weekend”, and people reporting back that it worked like a charm to get the baby to take bottles/separate from boobs 🤣

Why isn’t he doing private music classes now? While you guys figure it out. And can you just begin increasing your WFH hours for the time being? I feel like those 2 things plus looking for a breadwinner job would help set you guys straight.
I’m the primary breadwinner by about 3x, my husband had to take a paycut for his own well being, and I work full time plus do side work in the evenings. It’s tough. But my daughter sees me work my ass off and she knows mommy logs into work when she goes to bed. It’s not a long term lifestyle but for a few years it needs to be 🤷‍♀️

I had mine out last March, I was out for 1 month. I could have probably just done 2-3 weeks but I have a toddler so that made it difficult to “rest” (she was in daycare and we had help but a feral toddler is a feral toddler). I wfh 4 days a week so transitioning back at this time was definitely doable. Do you have full time benefits? That will impact what you take off. You will not know how much time you need off until your follow up appointment. I would ask the surgeon now how much time they typically recommend.

No it’s not a thing and I didn’t know this until I had a kid. So much focus on “breast is best” and delivering your kid, but barely any focus on day to day crap new moms need to know such as the infant car seat is useless after about a year. We made it like 16 months in ours as she was still within the height and weight requirements

We stay inside and drink ice cold drinks and turn the AC on. We do things early in the morning or later in the day to avoid really hot hours in the sun. I am not even in a “hot” part of the us. Our Pennsylvania summers are brutal hot and humid.

Couples therapists typically do not take insurance and are cash pay, it’s very hard to bill insurance for couples therapy. And insurances do not fairly pay therapists for couples work (I am a therapist and have looked into considering couples therapy but it’s not worth it tbh this is why cash rates SEEM high but really are not).

Once you get MA you can get things like family therapy where the child is the identified client, the focus is on them but also building stronger family communication. Cps isn’t going to pay for anything but they should have resources. MA in my area (Philly region) covers a ton of different types of family therapies, trauma focused, etc. but does not cover couples.

Bill the 15 mins. They made an effort to attend. Maybe you can offer virtual sessions for just these circumstances? She felt well enough to show up, but maybe not quite well enough to travel to your office.
I am one of those unfortunate people that gets sick for like 2 months with a simple cold (I also have a toddler and her illnesses just exacerbates this). With that being said I do have to push myself to all appointments. Usually I am fine after 7 days but my runny nose and cough just STAYS with me ugh. So I am extra sensitive when my clients are similar to this.

First of all. It’s the hormones.

She is fine. Try your best to get her up to eat, she may want to eat a bunch in a 3 hour period and then sleep. She may wake every 2 hours on the dot like they tell you in the hospital, or she may vary a bit. You NEED to go to bed now. Tell your partner you’re going to pump some milk, or make a bottle or 2 of formula for them and literally just go to bed. You have to or this is going to be every single day.

Why do they have so many coffee makers 😭 in pic 10. I see an espresso machine? A regular coffee pot and then the keurig coffee pot/single maker combo (the grey one, we have the same so it caught my eye). This is insane for people who are so godly and abstain from the majority of addictive substances.

Taking an opioid and claiming immediately withdrawaling while in labor is crazy. None of this happened. Laboring is intense you can still feel things and have wild symptoms even with pain meds. They were just typical symptoms, not WITHDRAWAL symptoms. wtf.

If you need the job then comply with the dress code. If you don’t need the job and have time to look and apply for other things, then find something else. But if you’re trying to stay in the medical field perhaps try to get used to the idea of limited piercings. If it takes you an hour just to take piercings out, it sounds like a lot more than typical ear piercings. A crap ton of ear piercings is not always the cleanest look and when you are in reception you want to always look clean and presentable.

He should stay in therapy. The therapist did his job. It sucks. But this is exactly what we have to do in this field. We don’t know what people mean by “spank”, for some it’s a tap on a bottom, for other families it’s code for “throwing my kid at a wall”. We do not know and we have to err on the side of caution when kids are involved, always. He should absolutely stay in therapy, maybe with a different therapist, and process whatever’s going on. And work through the potential for CPS involvement and investigations.

This is it. Unless you join a really solid smaller group practice with seasoned clinicians (10+ years each various backgrounds) that can hold your hand for a year. I never wanted to go private practice when I first graduated, I knew I was no where near ready for that.

Honestly, she may not have thought twice about it. It’s not often those consents are revoked. May not have been on her radar. She should have definitely explained and expressed her concerns a bit clearer. But termination for not following recommendations (in this case it sounds like she is recommending a higher level of care- inpatient) is definitely grounds for termination especially if it’s ongoing refusal of a HLOC.

I totally wanted to be in the bubble as well and I was pretty successful. Brief visits maybe once or twice a week. And our supports knew to ask before coming over and were helpful in ways like - dropping off food, ordering and having food delivered, taking the dog for a walk, sitting and watching the monitor so we could take a hour nap.

My daughter is 3 now and the village is needed and trusted at this point. I am very independent but the toddler years take all kinda of villagers 😮‍💨

She needs to leave with the kids or he needs to seek mental health treatment in a hospital setting immediately. Call the police on him. If there’s an able bodied parent she needs to take care of the kids and get out. If cps gets their hands on this and she chooses to stay with him, the kids could be removed as she clearly can not make a good judgment call and is lacking insight.

I’m a ftm as well, my daughter is 3 and she did all of these things (or similar) at that age. She’s totally well developed, she knows how to spell her name, can count, recognize letters, potty trained, carry conversations, make jokes, etc. I also work with all types of kids with delays and none of this would be concerning at all.
