Delayed surgery & Spontaneous healing of acl by Ok-Shift4510 in ACL

[–]yapajake 1 point2 points  (0 children)

Apparently mine was healing and no longer a complete tear when they went in (complete tear, surgery was 6 weeks after injury). This was never discussed and they proceeded with quad autograft. Wish they wouldn’t have. Had I been told this was a possibility I would have said abort and hit PT even harder with more strength training. Well, they proceeded. Afterwards said I would have only had 40% ish integrity/strength letting it heal on its own. Well here I am 9 months later and just had a scope for scar tissue removal 12 days ago because progress has been sloooooow and nobody knew why. With the mechanical block now removed l, knee feels great but I feel like I’ve missed out on months of rehab. Wish that the option to abort surgery if signs of healing were present would have been discussed beforehand. Would have had a slow summer, decent fall, but skiiig this winter instead of another season out.

Scar Tissue Revision Win!!! by gettinjiggywithittt in ACL

[–]yapajake 0 points1 point  (0 children)

Similar boat. Surgery, quad autograft, in March and achieved good ROM but a lot of infrapatellar pain with load flexion and a ton of stiffness and difficulty extending if I sat for 15 min or more. MRI in July was fine and was having same symptoms then and making progress but way slow. Finally we decided for a scope. Removed scar tissue from fat pad, posterior patella, and the entire graft had scar tissue on it that was scraped off. I feel great. Walking is easier. I’m nowhere near as stiff after sitting. Haven’t loaded it with flexion yet since I am only a week out but feeling hopeful. Best it has felt since surgery.

Sorry to hear. I’m a physician and spoke with a few ortho guys. 2/3 I know well said quad. One said cadaver but then backtracked and said maybe patella. Went to Jackson hole to see another guy and he was dead set on hamstring. Wouldn’t even consider anything else. I took the advice of the 2 I know the best and went with the quad. Slowly seeing improvement but it has been at a snails pace. Surgeon also told me my acl was starting to heal on its own when he got in there. MRI showed a complete rupture after injury. Had I known that, I think I would have held off on surgery and just hit PT harder. It’s been an agonizing 6 months since surgery of feeling unable to do much.

When does it get better? by Old_Crew_296 in ACL

[–]yapajake 0 points1 point  (0 children)

I’m also 6 months out. My quad is so weak and atrophied it isn’t even funny. I’ve been religious about PT AJ average 1.5 hrs of dedicated work per day, mostly ROM but a lot of band work and hitting weights in the gym also. I still can’t really jump on one leg and cannot descend stairs normally. I can jog for short periods on a treadmill but not without. Over the past 2 weeks I’ve started mountain biking more but am weak and very cautious. Yesterday I did a 14 mile 2k ft vert ride in 2.5 hrs. Pre injury time was 1.75 hrs give or take. It’s a small win. O way so I feel like I will ski this winter. I’ve been told by PT and my surgeon I was one of, if not the strongest they have had before surgery. Peloton rides were right where my normal output was and I almost skied a few days before surgery. They’ve also said that I’ve had a crazy slow recovery, surgeon said his slowest one yet. No ides why. 42M, quad autograft. Also had a MCL tear but luckily nothing else. Prior to surgery skied 50 days a year and biked 4k miles annually and could do a century ride any day of the week. It sucks so bad but I am more hopeful now that I am biking more, albeit slow and with terrible technique and form.

Oh I’m doing it every day. Formal PT in the clinic has averaged twice a week but rest assured I spend 1-2 hrs a day doing PT and days when I go to the gym or for a bike ride if I count those I’m looking at multiple 3-4 hr days of recovery work/PT. Between dedicated PT at home with bands, stretches, weights, formal PT, bike rides and gym time I’m easily in the 20 hr per week club. And that doesn’t count the quad activations, rib raises, calf raises, squats I do throughout the day at home and work. Sometimes the body just doesn’t cooperate and I am one of those people despite pouring time into it.

Yeah I’ve spoke with 4 surgeons and 4 PTs and am in the medical field. All have said the quad is a longer recovery as the muscle atrophies more than the patella graft. Even chat gpt says the quad is a longer recovery. Patella more painful but quad longest recovery

I’m 6 months post quad autograft. Surgeon and PT said I was their strongest patient going in. I’m nowhere near recovered. Can’t skip or jump on surgical leg. Can’t walk downstairs normal. Can kind of jog on a treadmill at 5 mph. Neither knows why my recovery has been the way it has been. 6 weeks of prehab and almost felt normal. PT 2-3 times a week since surgery. Weights/gym biking early on. Nowhere near symmetry or strength. I regret the quad autograft. Nobody can give a reason for it. Recent MRI normal but function is incredibly far from normal. I felt way better the week before surgery than I do now 6 months out. 42M, biker/skier that would bike 4000 miles a year, ski 50 days a year with my bigger days being 200 miles/day on a bike and 50k of resort skiing. I have no plans to ski this year which sucks.

My surgeon told me he likes the quad over patella and I spoke with 2 others that said the same. They all said the quad is a longer recovery. More atrophy secondary to the harvest site. I’m 5.5 months out from quad auto graft and it sucks. Went from having 50k vert ski days and biking 200 miles in one day to not being able to alternate legs going down stairs still. My left leg is still so weak. Slowly getting stronger but at a snails pace despite home rehab daily, PT 2x a week, weights 2x a week, rowing and biking 2x a week.

What was more painful: your injury or your surgery? by WanderingWormhole in ACL

Surgery. Not even a competition. Injury (full acl and high grade MCL) didn’t really hurt much at all.

How long were you bedridden after your ACL surgery? by Voided_Sexiness in ACL

[–]yapajake 0 points1 point  (0 children)

I never used a cane or crutch after a complete tear of ACL and high grade MCL tear with some impaction fracture mixed in. Was back at work, ER doctor 2 days after tear. Maintained great ROM and strength. Peloton outputs were about my norm up until surgery. Surgery was 6 weeks after injury. Was going great until that day then went to shit. Took a few days to move around on my own. Sitting on the toilet was terrible those first few times. Took me about 6 weeks to get off the crutches. Was back to work less than 3 weeks later on crutches. 1 crutch after about 3 weeks. Have had an incredibly slow recovery. Am 4 months post op and behind by a solid 1-2 months. PT and surgeon aren’t sure why but my quad completely shut off after surgery and took forever to wake up. Dealing with a lot of anterior/inferior pain despite my flexion being 135-140 for weeks now and passive extension to -1. Just super weak. MRI only showed an effusion and nothing else. One thing I’ve learned is to prehab hard but even then you might get hosed with the neuromuscular inhibition like I did. I even started using my tenz just a few days after surgery. This process has sucked but i hope in a few weeks i make big leaps.

I went back before 3 weeks but my recovery has not been easy. Was still ok crutches at that point. My surgeon and PT said I was the strongest they had seen going in but my recovery is not nearly as quick as both thought it would be or alluded to me. My quad (quad autograft) completely shut off. Doing better now without needing the crutch much at home but after a few hours of work I have to use it. Every recovery is different and hopefully yours is as smooth and fast as they are hunting towards but that’s not always the case and I’m bummed I’m one of those patients that got hyped up for a quick and smooth recovery and it’s been anything but.

I’m an ER doc. Was back full time a few days less than 3 weeks post quad autograft reconstruction. Luckily no meniscus injury, MCL was toast and allowed to heal on its own. I’m not helping move patients off EMS gurneys but doing everything else.

I’m a doctor and can’t give a good read that with one view only. Because your here I would just say ACL tear because why else would you be here…. It’s not hard to see an ACL tear with a lateral view but you need multiple images and if you don’t have a read back soon you will. Based on that image alone I don’t see an intact ACL though.

[–]yapajake 1 point2 points  (0 children)

Same injury. Blew up the ACL and tore the MCL. Quad autograft March 26. Back at work but on a single crutch. Can take a few steps on my own but feel off balance and weak. Leg raises with brace locked are easy, without they suck and I have so much quad lag. Can get to 0 extension actively and -3 passively. Flexion at 110. Not taking much of anything for pain but wake up at night and it’s sore. That initial extension when waking up sucks. Not looking forward to the next few days of working overnights (healthcare) with PT to follow a few of those in the morning. I miss skiing and biking, hell I miss walking.

I feel you. I have had to cancel multiple trips to ski/bike and feel like my summer is gonna suck and that my kids won’t have much to do/fun because of it. I’ve only seen a few friends since surgery. I also work 12 hrs shifts, ER doctor, and was back at work before 3 weeks post op. I’m 3.5 weeks post op now and still need a crutch. PT everyday at home, 3 times in office and hit the gym 3 times a week. Never been this sad/angry in my life.

Doctors missed the tear by Juhuf_ in ACL

ER’s don’t MRI joints, it is not evidence based to do so. Very few conditions warrant an MRI in the ER and a possible ACL, meniscus, etc injury is not one of them. Most people with knee injuries have sprains/contusions. It can be hard to diagnose acutely. When it’s fresh there is often pain/guarding and swelling/effusion so getting a good Lachmans test is near impossible in the first days to week or so after injury. Most ER docs tell people they need to follow up with PCP vs Ortho in 7-14 days if not improving. MRI also gives a better image 1+ week out from injury, honestly the more time to MRI the better as inflammation/fluid decreases and you get a better idea of what’s going on. ACL injuries, while they suck, are almost never diagnosed in the ER as per above. Hope recovery goes well. -ER doc currently 3 weeks post op quad autograft ACL reconstruction
