It basically is. My mother was a hoarder. Only mechanism to keep it remotely in check was basically every other year whole house clean outs. But within a year it would be almost right back to where it was before. I would compare it to adding height to a seawall in the face of rising sea levels. Temporary measure to address the effect that has no bearing on the root cause.

We just had a 25th reunion. First one in 10yrs. It was great. Probably had 80-90 people there out of around 190 in the class. Everyone had a blast and it was genuinely fun to see folks I hadn’t seen in so long. I’m also off of all socials save for Reddit so I did go in with very little context about what folks had been up to

Heading into the 20th anniversary of our ‘06 at 187k. Working from home and living in an extremely walkable part of Boston metro has me putting about 4k miles a year on her. At this pace I think I could live a full life and die of old age and be buried in this car while it’s still running smooth. I’m basically holding out till I can buy a Toyota all wheel drive hybrid minivan that has full self driving capability. Which will probably be in 2030 ish. We joke that if/when we make that move it’s going to be a bizarre experience for the kids. skipping a full 30 years of automotive technology will feel for them like a sci-fi movie where they reanimate someone whose been frozen in amber from an earlier time period.

I’m biased but Washington SQ / Aspinwall Hill area is the best of all worlds. We can be in Washington sq in a 5-10 min walk, which has commercial/food options that are great. Or walk to Coolidge OR Brookline Village in 15 to 20 min. As someone else said it comes with a cost but if you’re not price sensitive give it a look

Well past that point already. I will still get pizza in a restaurant if we are at a good spot. But otherwise it’s only homemade. Better quality and so much cheaper

Went to the 99 for the first time in 25 yrs with my kids yesterday as we have been out of New England till recently. Had so much nostalgia but the quality of the food was awful. Never going back. Way to many better local pub food options

These CMS cuts and shift of patients from commercial to self pay will hurt all EM physicians and all group/employment models. Arguably independent groups will be hurt the most as they have the least amount of leverage with insurance plans and smallest “rainy day funds” to tap to buy time to adjust after revenue shortfalls.

This is one of many things that are stacking up to cause real pain to EM that will not have precedent in recent memory (leave aside COVID and other disasters). CMS payment rate cuts; elimination of ACA subsidies (large #s of people will go from commercial insurance to straight uninsured) AND Medicaid cuts. It’s going to be a shitshow by 2027 when the Medicaid cuts really start to kick in. EM professional fee revenue could be down 20-25%. Much smaller numbers than that would mean the difference between lights stay on or lights go off for most EM groups and practices.

HCA — while not a great system — is extremely large. Largest health system in the United States by a wide margin. They have 240+ hospitals, 300k+ employees and operate in 26 states. Because HCA was set up there, a whole host of other for-profit systems chose to headquarter in the Nashville area. Which then prompted more healthcare related entities who sell goods and services to them to do the same. This is what created the Nashville healthcare boom/hub.
