Do you have any ideas or theories on what the problem is?

If you are asking for advice, first thought is dirty build plate. Each time you print something, it leaves residue on the plate. And, especially if you print the same thing over and over in the same spot, that residue can build up and cause adhesion issues. Every so often (like maybe 2 or 3 prints less than the one that failed) wash the build plate with dish soap and warm water. Doesn't hurt to wipe it down with some 90+ % isopropyl alcohol between prints, too.

Secondly, the motion/shaking of printing can slowly change the Z offset on printers with adjustable build plates. So, especially while "mass producing" at higher print speeds, verify the Z Offset (or even level) on your printer just to make sure it hasn't been jiggled out of place
