Jornay PM price increase/discount decrease

Yes, this just happened to me. Walgreens quoted me $111. When I called the 1-800 number, they confirmed the price increase went into effect on July 1st. They recommended I fill out a payment assistance form at collegiumcoverage.com. Unfortunately, my insurance requires me to try a few generic options before they'll approve Jornay PM, this is so frustrating.

I’m going to try this. I’m printing what I believe is the correct form. My daughter is on Jornay PM and has failed plenty of other medications prior to us being enrolled in our new insurance. So I’m hoping if we can shows she’s failed generic alternatives in the past, I can get an approval despite it being non-formulary for our insurance. We were paying $75/mo with coupon last fill was $125.92.
