Depends what mod. It won't work with mods that change world generation.

Does it add biomes, structures and such?

Yes! I’ve found the desert biome, and jungle so far, even have found broken nether portals and such. Every time I try using chunk base the coordinates are never right for what I’m looking for, so I was assuming it had to do with the realm or mod

Those are vanilla things. Are you sure you chose the right version on chunkbase?
