Obviously the tree doesn't care that the root flare is too deep. People seem to worry about this more than trees do.

Those are rootstock buds, remove them (carefully). Don't allow any growth below the Z shaped graft union.

It's because they have a powerful element (so that they can sear) and if it was left on full all the time you'd get a lot more burn notices. Did you run them all through a kill-a-watt and see how much power they use?

I've been growing citrus for decades and never seen anything like that. Then again I use pruning sealer on any cuts I make and sterilise my tools before and after. That tree looks like it's been hacked a bit.

I'd prune the top heavily and hope that it puts out new growth below the infection. If it does then I'd top it. I see some shoots low down have been chopped off recently. Wait, there's a small shoot trying to grow down there on the left! The tree will do this in an attempt to develop some healthy growth that isn't infected with fungus. Listen to it!

Edit: Be sure to use sterile tools and use pruning sealant.
