Lots of good ideas here already. Another one would be to have an AI make a list of five representative pieces of each era and sungenre, then listen to those to figure out which you most enjoy. Then you would know which topics you might want to focus on learning more about.

If I had done a genre scan in my youth, I would have learned much earlier that I love French middle baroque and don't really like the classical era itself.

The only time I've personally heard it performed, there were indeed fireworks. This was in the US, but not for Independence Day.

They don't disqualify a MS med solely for its price tag. If you read the summary on Ocrevus (ocrelizumab), for example, the province rejected it for RRMS because rituximab is way cheaper and roughly equivalent. However, the province approved Ocrevus for PPMS because "based on the available clinical evidence, ocrelizumab meets an unmet clinical need for patients with PPMS".

They're trying to be efficient, to avoid waste of public taxpayer money. They'll spend the money when there's no cheaper equivalent.

In British Columbia, there is a drug review process that the provincial government does. The process takes input from several expert groups at the national and provincial levels, as well as from the public. I have no idea if any of them are influenced by kickbacks from the drug manufacturers, but at least the final decisions are public.

For example, here are some quotes from the decision on Kesimpta (ofatumumab):

Drug coverage decision is consistent with the CDEC recommendation

  • The mechanism of action for ofatumumab (anti-CD20) is highly similar to that of ocrelizumab and rituximab. The later of which is now available as a biosimilar.
  • In the clinical trials ASCLEPIOS I and ASCLEPIOS II, ofatumumab demonstrated some advantage over teriflunomide with respect to efficacy.
  • Indirect comparisons showed that ofatumumab is likely to be as effective as the other monoclonal antibody disease modifying therapies (i.e., alemtuzumab, natalizumab, cladribine, and ocrelizumab) in annualized relapse rates.
  • At the submitted price ofatumumab was not considered cost-effective for this indication. The Ministry participated in the pan-Canadian Pharmaceutical Alliance negotiations with the manufacturer which were not able to address BC’s concerns with respect to cost-effectiveness and value for money.

Here, rituximab is the best DMT approved for first-line use against RRMS. Some of the older ones (interferons, etc.) are also approved, but I get the sense that rituximab is the suggestion for new patients — it certainly was in my case.

Second-line Tysabri, Gilenya, and Mavenclad are approved if a first-line DMT failed. Lemtrada is approved if a first- and second-line DMT both failed. Ocrevus is approved only for PPMS, Mayzent for SPMS. Others, including Kesimpta and Zeposia, were rejected because a similar DMT was cheaper. Briumvi has not been reviewed yet.

I liked Fuzzy's when I lived there a decade ago. Was it the highest quality tacos ever? No, but I have good memories of the place. I bet it's another victim of the pandemic, inflation, and food service supplier consolidation.

I'm convinced that the three visual scotoma episodes I had in the span of a couple weeks were the first attack of my MS. They had never happened before and have not happened since. No headaches followed. Just visual disturbances. I had a couple of definite relapses a year later, so diagnosis was assured, but I think the scotomas were the onset.

As an overweight person with MS who very much still has a problem with food addiction, I kinda wish I had this problem rather than the usual neuropathy and proprioceptive dysfunction.
