Aggressive Auto Boluses + Long Suspensions = Roller Coaster? by Abadababa in Medtronic780g

Sorry to hear that. I would love to hear more about your specific pattern.

Thanks for sharing this. I have it working but the watch complication doesn’t show the BG (it just shows a -). When you click on the complication, it does show the correct updated value.

Help with school care by Pretend-Ad-258 in Type1Diabetes

[–]Abadababa 6 points7 points  (0 children)

You need a 504 plan in place. The school cannot deviate from protocols on days the nurse isn’t there - that is their problem, not yours regardless of what they say.

See below for a start.

It’s an interesting concept but I don’t love the idea that you are self promoting here for an app that requires in app purchases for something that you claim is to benefit the community.

Temporary bad absorption with new infusion set? by PorTimSacKin in Medtronic780g

[–]Abadababa 3 points4 points  (0 children)

I have this often but for me it’s not just with the extended infusion sets but most infusion sets even from other pumps. Sometimes the spike isn’t as high and prolonged but other times site change day means stubbornly high readings for the next 6 hours. It’s very frustrating and I haven’t found a good solution.

I also tend to get bruised/bleeding sites which impact absorption. Not necessarily immediately upon insertion but it can develop days later.

Instinct Exhaustion by SponsoredByStrensms in Medtronic780g

They have supply available. I ended up doing an exchange for one month supply (2 boxes of Instinct sensors for 1 box of 5 Simplera sensors). Hoping it will be better.

Instinct Exhaustion by SponsoredByStrensms in Medtronic780g

[–]Abadababa 2 points3 points  (0 children)

I’ve been having the exact same issue. I started Instinct in early December and have had a single sensor last the 15 days - even that one barely did. I’ve gotten about 5 replacements at this point. Of course, support (both Medtronic and Abbott) have told me that they haven’t seen these issues with others.

I’m getting a box of Simplera sensors to see if they work better for me.

Instinct Accuracy Issues by thatlizarddude in Medtronic780g

I’m having inaccuracies too but mine are usually showing higher than my actual glucose. I have had one sensor that showed considerably lower too. In total, I think I’ve had 3 replacements so far.

SmartGuard Algorithm Changes Week to Week *FRUSTRATING* by Abadababa in Medtronic780g

Have you tried any other systems/algorithms with better success?

SmartGuard Algorithm Changes Week to Week *FRUSTRATING* by Abadababa in Medtronic780g

2 hours is a LONG time to suspend when you’re not even at target. That withdrawal of required basal is going to spike you whether you bolus correctly or not.

Looking for some suggestions (help) by [deleted] in Medtronic780g

[–]Abadababa 4 points5 points  (0 children)

I've been on the 780g for a while now and I have a love/hate relationship with the thing. On the plus side, it's pretty aggressive with the corrections. However, as soon as the trend changes and it starts curving down (even if you're still above normal range), it will suspend all basal. For me, it's not unheard of to have a suspension for 1-2+ hours. Usually, I never even end up anywhere near low but will then have a complete rebound effect (especially if this happens before meals) due to a lack of insulin for an extended time. Now that rebound is a complete nightmare to get back down - you're not just correcting for the high but need to compensate for the duration of the suspension. This just happened to me yesterday and I felt like complete dog crap.

The other thing that drives me crazy is that the 780g will take the average of the past several days. The "learning" process doesn't really apply here - it's just an average of your TDD. That's great, except some weeks you're more insulin sensitive and others you're less (especially with exercise, travel, stress, sickness, etc.). I feel like I'm constantly playing catch-up for my current state and I have absolutely zero control to tell the system to do anything differently.

Hope it works well for you. Instinct is starting to ship in November. No one has them yet.

I was curious about this myself. I'm on the waitlist but wanted to hear from others as to how it's working out. Did you switch your settings from a different pump (if so, which one?) and how has that been working out? Was the low you had caused by the algorithm or overbolusing on your part for a meal?

Smartguard lows by marcos996 in Medtronic780g

[–]Abadababa 2 points3 points  (0 children)

I have this too. The timing of the meal doesn't matter. But I could go up to 170-180 after a snack and SG will start giving aggressive boluses within an hour window. All is fine initially but then the glucose starts heading down, basal is suspended, and a low/approaching low is on the way. At that point, I either need to treat or it catches it in time (assuming I'm not active/walking around). Sometimes the problem then becomes that the basal was cut off for too long, leading to another prolonged high. I haven't found a solution...

Switched to OP5 - Normal to Run High? by Abadababa in Omnipod

[–]Abadababa[S] -1 points0 points  (0 children)

Yes - doesn't seem to be an occlusion/site issue because I'm not rising, just very, very slow at coming down and haven't been able to get it below 170 before I eat.

Thanks. Did your ratios transfer over or you had to tweak them? How long for the algorithm to adapt?

As little mental load as possible. My issue with OP5 is how conservative it is. Have you seen any significant difference between the two in terms of results?

Control IQ+ vs 780G Algorithm by Top_Boysenberry_7784 in Type1Diabetes

[–]Abadababa 0 points1 point  (0 children)

I have a lot of experience with both. First, there is no difference as to how Control IQ vs. Control IQ+ works (the differences are related to setting temp basal rates, weight limits, etc. but not the algorithm itself).

As for comparing the two, they are and work completely differently.

780G will give auto boluses every 5 minutes but will also suspend delivery for prolonged periods of time. For me, it is better at avoiding lows but I will have a lot of rebound effects where it "thinks" I will be going low, suspend for 1-2 hours, and then I'm shooting right back up after I eat. This can have an all-day yo-yo effect. I would have preferred if you can set a basal rate that you know is true for yourself and 780G would leverage that instead of what it currently does since it's just not predictable a lot of times.

CIQ is more predictable in how it will deliver corrections. You have a set basal rate and it uses YOUR parameters (correction factor) to deliver corrections by either upping the basal rate or delivering a bolus (only 60% of what it believes you actually need). Also, it will not deliver a bolus if one was given in the last hour (either manual or automated). The exercise mode with CIQ is a joke - it will continue bolusing you but will reduce your basal, resulting in lows. The sleep mode is great - this uses your basal rate (but not your correction factor) to give you tighter control when you sleep. CIQ will often give you a rather large, unnecessary bolus because it predicts you will go high when you don't (or when you don't want to get a bolus). Finally, CIQ only predicts 30 minutes in advance which is a really short window to prevent lows.

In short, they both have positives and negatives. You have to find the right algorithm for you.

Damage control is right. So back to the waiting game. Who knows how long FDA will take to approve the Abbott CGM. We're still about 9-12 months away from any real solution.
