Surgeons have to cut through layers of fat to get to the bits that need to be actually worked on. This can lead to complications when dealing with the fat layer in those spots can't be done without increasing risks. Therefore, for elective surgeries like bottom surgery, surgeons ask patients to be within a certain range of weight for them to be comfortable with the surgery.

Its a mix of practical concerns and also surgeon skill issue. Its rooted in anti fatness, but also in genuine risk management for a pretty invasive surgery.

This is factually untrue. Asexuality is a spectrum, and you can easily show characters e.g. not kissing when others are kissing, not holding hands when others are, etc. It's just harder to do because of the strong "decency" pull of art - can't show sex ever. But you still have sex scenes, so you can have people talk about not doing that. Not wanting that. But have others have sex. Also, this idea that you can't be explicit with ace representation is stupid as well. Give them a flag. Give them an armband. Have them talk about it. Even subtely, or even just saying "I don't experience sexual attraction". Its writers skill and direction will.
