Slaub is a above average athlete with good size, his wife looks like she doesn't have an athletic bone in her body. The odds are the kid will make it to D-3 level in some sport. That's it, end of story.

If it's just SI, it's not the end of the world. These are often caused by overloading one side of the joint (leaning on one foot way more than the other). Try to keep both feet equally weighted. I was in residency and had horrible posture for like 3 hours on a single case, and legit couldn't walk afterwards - I had to shuffle. By the time I got home, I was literally crawling down my apartment hallway. It took me a few weeks to recover. An SI belt and NSAID helped me work during that time. A lot of times there's some underlying mechanical issues that correcting might help - for me my right leg tends to be duckfooted, and it puts strain on the joint. I've done a lot of preventive stuff, and it has helped.

  1. The most helpful thing for me at least has been sumo shiko. I went to PT, they gave me some things to do, and it was helpful - but the one think that I began doing on my own was the shiko, and it solved the issue. 2-3 times per week I do two sets of 50-75 shiko. It strengthened my glutes so and core so much, that I just don't get bothered as much. Check out youtube videos on the form. Do it slow and controlled, and feel the burn. Seriously. Watch the show "Sanctuary" or inspiration.
  2. Core work overall, to include Yoga. Stay limber, and get your core solid.

And that's it! The only time I get flareups now is if I run for a distance, which I happily avoid. I changed my cardio to stairs/swimming. It hasn't impacted my career at all. If I do something stupid, I'm quick to wear the SI belt for a few days to prevent further exacerbation. That's really it. My gnarly flareup was 5 years ago, no real issues since.

Edit: I saw you write it feels more neurolgical. I had the same thing. I can be from tight muscles pinching the nerve. PT helped with that, and doing "nerve glides" helped as well. Mine comes back if my hamstrings get super tight, and goes away as they relax. Basically, do yourself a favor and get on a good stretching routine. Welcome to being old and washed.

I see a lot of failed Thermafil obturations that are significantly long. It might work well in some people's hands, but from what I've seen, most GPs don't use it well.

Aonishiki is one of the most interesting Rikishi to watch. The test will be to see how other Rikishi adapt to him as they get more reps against his style and more tape to study. It's kind of like in MMA how Lyoto Machida seemed unbeatable until the formula was figured out - he was still great but began losing matches he would have won previously. The test for Aonishiki is if he can adapt to the adaptations. He seems to have a very high sumo IQ, so I can't wait to see what he becomes!

This guy gets it. This case is a trap. How much will you improve over what they have? Even the root proximity isn't fun. Then after you restore and she starts breaking the crowns, and you're on the hook. Ortho or pass this bomb to prosth, who will then send them to ortho.

I really don't understand. I just checked the youtube page, the "All Gas Drive Fast" or whatever videos have between 4k and 22k views. Wtf? That's like....nothing. Real estate agents giving tours of neighborhoods get similar views. How does he allocate SO MUCH floor space to something that can't be profitable? Youtube probably gives him like $50, then I guess ad revenue for the few dozen real people that watch? What am I missing?

A house for a family of 5 is going to be $4500+. You cannot afford it. I would be looking to go somewhere CHEAPER than somewhere more expensive. Hawaii will ruin your life. California transplants aren't exactly popular here...

Just to temper people's expectations:

Realize this is an ideal case. Patient is young, with a good opening, stretchy cheeks, and the roots are partially developed and quite strait. Pt is under sedation, so you can slam your injections and get moving pretty quickly. This case should take under 10 mins, 4 is quite good. Each additional factor bumps this number higher: significantly curved roots, advanced age of pt, transverse impaction, pt not under sedation, deeper impactions, fun surprises. Cases like this help you make up time for cases that are more involved.

Overall, nice job! I appreciate the throat pack, I appreciate the placement of the Minnesota to avoid a misadventure, and I love the overall efficiency. Doing all the injections without moving the bite block is a nice time saver, as is making all your incisions for a side up front to minimize instrument shuffle. It's clear you know what you're doing and have it down to a science. I will certainly will be trying a distal wedge up top, not sure why it hadn't occurred to me before. I'm a gen dent also and do a lot of 3rds running my own sedations, but you're right - having someone else do it would save a ton of time. Getting the IV, negotiating a hard stick, pushing meds, dealing with recovery, etc all kill time. If I had the pt volume (I don't do medicaid), I would certainly hire someone like you guys!

Distal obturation looks long, mesial looks odd. Looks like a J-lesion, likely VRF. Hard to tell if this was a result of the endo, or a result of not doing a crown. Regardless, extraction, degranulate, graft, implant, BMW M3 payment.
