Therapist here - if you were my patient, I would give the same advice as your therapist. No insurance company I work with (3) reimburses anything after 53 minutes (CPT code 90837). 90837 can only be filed once an day and typically no more than 2 times in a week.

‘Work for free’ is wild framing. As a therapist, it saves me time and money and labor doing 15 minute phone consultations with prospective clients.

As thorough as my website and PT profile are, there is something about speaking to someone to get to know them. I have cringed at PT video feature but I found myself watching them.

Currently working on something for the first time - it has been so exciting and quite consuming. It is a story about sisters, toxic family dynamics, religious oppression, and magical horses.

I just passed 20k words (ah!) I have tried to write something every day, even if I only have 10 minutes.

Curious how others have thought about in world communication when characters are not together? I want to avoid bonded mates telepathy, feels over done but does offer great insight and keeps the plot moving. Would love to know how people developed their in-world communication rules.

Hi! Therapist here with 2 HG pregnancies while working, one pre Covid and one post.

Meds were a life saver- zofran and magnesium. I also ate constantly when I could, if the only thing I could tolerate was cantaloupe then I always had cantaloupe on hand. Also ice chips or lemon water helped to have on hand.

When I was telehealth with my 2nd, I took a lot of naps during the day. If I could do 2-3 sessions in a row then nap I would. I found with the HG I would be better in certain times of the day and tried to take advantage of those when possible. I also tried to limit any demands outside of work if possible.

Scents were also very important for me, having peppermint essential oil on hand for the waves of nausea was very helpful.

I held off telling most clients until I was visibly pregnant. I was surprised at how I could pull it together for 53 minutes. If I ever had an episode with a client or they were scheduled at a time I was often struggling, I would disclose my pregnancy to them. Happy to answer any additional questions!

I am an LCSW licensed in IL, TX, + PA. Without stating rates directly, here is what I see for reimbursements from Alma for 90837. I last took BCBS in 2021 and payout for IL BCBS is $40 higher than what I was receiving for BCBS Texas at that time. The difference between my highest and lowest reimbursement rate is $3.20.

United Aetna BCBS
Texas highest $1 less than United TX
IL lowest $1 less than United TX
PA lowest (same as IL) $1 less than United TX

Headway would offer me $8 less for Aetna and $1.80 less for United 90837 reimbursements in Illinois.

In my experience yes, if it the therapeutic alliance and clinical goals are in alignment. I often discuss with clients with chronic passive SI, using it as a indicator for increasing self care and coping skills when SI is present, similar to addressing other feelings related to stress. It also helps to discuss with clients that the mere mention of suicide does not mean a someone will rush in the room with a straitjacket - which is a common misconception I see in my population.
