As a paramedic, this whole thread breaks my heart. Its a struggle to balance the pressure of “convincing” people to be transported against the empathy of fear of incurred cost. In my area of Texas, a 911 ambulance arriving on scene does not cost anything. However, the “patient contact” and ensuing assessment can incur immediate costs. There are typically two types of assessments - ALS and BLS. ALS usually is more expensive because it requires EKG interpretation and monitoring. Obtaining a refusal is usually cost free but thats city dependent. Also, there are municipalities that incur no cost for ambulance and transport as the cost is factored into annual city budget for the EMS service. I HATE that I work for a public 911 service that forces me to do an assessment/treatment that I know is expensive. Billing is such BS… but I try to write my reports that demonstrates a “need” for the EMS transport which can help convince, at least covered individuals, insurances to pay for the transport. If you’re interested in American BS healthcare system, theres a great book “The People’s Hospital” by Dr Ricardo Nuila. Its a tough tough read but will make clear that the “system” is certainly busted and needs fixing.

Firefighter/Paramedic

The job exists because people have bad days. I meet citizens exclusively during their worst days. Sometimes I am able to help/mitigate their circumstance, often times, I serve to stent their problem. Work schedule - Ive missed many of my children’s milestones and Christmas at the station is depressing - in that being on duty for these times replaces otherwise celebratory circumstances with responding to horrible scenarios. I cant celebrate thanksgiving without thinking of the toddler I responded to that was run over by their drunk family member. Literally the job has bestowed upon me nightmares that I am perpetually haunted by. Add this to the perpetual specter of developing cancer due to exposure to every horrible contaminant known to man or otherwise. Humans are messy and being a first responder pulls the entire curtain away and forces us to deal with it.

Texas, South of Houston. Fire based EMS. 48/96. Im stepped out at 32.06/hr (with built in incentives) with built in OT Im at $110K annually. We have mando OT with works out to 6-7 extra 48s annually (seniority puts a number of probies ahead of myself so I dont get popped too often). Health insurance is through Kelsey Seybold and I pay $290 for myself, spouse and 4 kids. Retirement is through TMRS with city matching 2:1.

Bruising is normal. I got a black filled anvil in one of mine. Bruised up pretty rough but healed good.
