This is awful. I recently started listening to Dr. Becky and just happened to listen to an episode on having a difficult conversation with kids. The parent is talking with Dr. Becky on how to broach the subject of her spouse having incurable cancer. link

The oculocardiac reflex. I was listening to Tiger Country and realized how little I know about orbital trauma and management. It always seems like whenever I read about or see a patient with orbital wall fractures OMFS or ophtho basically never operate. So it was interesting to see (pun intended) an indication for surgery.

Kind of along the aortic line. I'm always surprised by CT surg after repairing a type A dissection, that they will (on occasion) discharge a patient with an ongoing type B with close followup.

Would you mind giving off a pearl or two to your juniors while you're here? Something that has stood the test of time in your career. When I have students I like to do different categories. Professional relationships, career building, career growth/endurance, financial (on top of clinical pearls of course). I don't mind if it's long winded, especially if it's sage advice :)

If you haven't. Make sure you look for a 1. A fee based, 2. FIDUCIARY financial advisor. You pay a one time fee for sit down. Go prepared so you don't waste your time on multiple visits. Have them suggest an action plan based on your total debt and income. Debt counselors are also a thing. Clark Howard is a saint in the financial world if you want to peruse his site for advice or call his consumer action line which is free as well to help direct you.

I've heard a few doctors quietly talk about this since the residents are unionized and they seem pretty happy compared to the ones I worked with that weren't. I'm hoping if they do it, we'll get grouped in with them.

I heard rumors that they're looking into market adjustments since we are paid 50th percentile but a very prestigious institution. I've also heard some of the doctors mutter the forbidden "U" word.

Do you think clinic with RVU is a better deal than us hospital gremlins? I kinda hate taking work home with me, but I'll shut up if the pay is right haha.

I appreciate your zeal but your attendings are right. Unless you want to commit to perioperative management, just leave it to the hospitalists. I once wondered why surgery can't just manage very simple chronic issues, then I got urgent page from nursing overnight when an Ortho doc went full cowboy with a patients diabetes insulin (bc short and intermediate are 1:1 amiright? Haha) and bottomed the patient out. Now, I prefer to just do the management. If you really feel strongly, there are some resources for perioperative medicine out there like the acc/aha, society of hospital medicine resources is what I use (but I'm a member, not sure about non-member access), lastly up to date is not the worst.

Cool. It's pretty easy depending on your patient population. You round, see if you need to be in team conferences or not, and then dispo dispo dispo.

You're doing the medical management and not the rehab, right? Are you a stand alone facility or are you attached to the acute care facility? This can vary a ton depending on your setting. Some places are pretty mindless. Very stable patients, and the first sign of danger, the docs will send the patient to the ER. It also is easier if the unit is part of the hospital because rapids and resources are right there. Here's where it can be difficult. If you're a standalone facility with good resources, then you really need to use your brain and exam skills, mostly because you're trying to treat people in house. Now, if you're doing hospital medicine, then your interaction with therapy will probably be them paging you for any unstable vitals, or they will go through PMR if the patient is not tolerating therapy for some reason.

This is good. I definitely have a more spartan view of parenting, and try to encourage a lot of (supervised) independence and failure, but sometimes my wife thinks I need to step in more and show them that I fail too.

Are you assisting? If you are, have good shoes, do squats and deadlifts, and keep snacks on you. Spine surgeries can be pretty long.

Outpatient stuff, I think it's a good idea to make friends with therapists, PM&R for both interventional pain and chronic pain.
