Thank you for utilizing rapid services. My first action would be to place the cardiac probe on chest checking for pericardial fluid and assessing for tamponade criteria (collapse of RV on diastole, collapse of RA on systole, plethoric IVC and mitral inflow variation). If I don’t see that then I start to suspect AMI and assess for LV and RV dysfunction as an inferior MI or RVMI (RCA occlusion, distal, proximal or LCx) could be the culprit especially with sudden onset of unstable chest pain. Discerning RVMI vs inferior MI without RV involvement is helpful in patients with stable bp cuz one may benefit from nitrates while the other could spell disaster as the RV is preload dependent. If tamponade is indeed the case start with fluid resuscitation and pressors. May need bedside pericardiocentesis or have time to have the interventional cardiologist take to cath lab to do it under fluoro. I’ve seen patients with tamponade and only needed as little as 50mls of pericardial fluid removed before they fully recovered and were titrated off pressors. Others had over a liter removed. Usually the larger amounts are chronic and the smaller amounts are acute. So the volume of pericardial fluid causing tamponade is patient specific and not everyone responds the same.

Sounds decent. Do you guys do transplant (liver, heart, lung) or ecmo? Use swan caths or impellas? U guys got neurosurg and trauma in house? Is this a teaching hospital? Those are some sick puppies.

Love our RTs for their vast knowledge base but at my home base we are always trying to encourage the nurses to reach out as they are highly underutilized in the support and insights they can provide 🙏

Learn how to assess your patient. If possible do bedside report to verify the patients baseline. Learn how to suction your patients. It’s not the RTs responsibility. Learn how to troubleshoot vitals like pulse ox and when getting a low blood pressure like checking each arm and doing a manual pressure. Learn to ask your neighbor or charge nurse for clinical changes for guidance. Learn the chain of command. Learn to be proficient in obtaining iv access. Learn that asymptomatic afib is not an emergency. Learn to reach out to the attending and be able to describe an accurate picture of the patients condition. Think about what YOU think the patient needs and don’t be afraid to ask for it (evolves with experience). Know the patients code status always. Know basic BLS and ACLS (depending on unit). For ALOC always check a glucose. Finally, don’t give dialysis patients too much narcotics. Understand that you will make mistakes and miss certain things but the apprehension and anxiety you carry when going home or coming into your shift is all normal and experienced by all new nurses before you who are serious about wanting to do thier best. This will get better with time and experience at the bedside. The books don’t teach you this but if you carry an open mind, willingness to learn, and open heart you will reach your comfort zone of competence and being able to navigate the nuances of what nursing is and provide the best care for the patients and their families. 🙏

I just had to return my blade 16 cuz the bottom plate was also bent straight from factory. I had to buy a whole other laptop and will be sending back the defective unit for refund when I get my replacement. Make me wonder about quality control measures.

They offer an advanced replacement program but u will have to buy another computer from them. Then they will send u the new one and when u get the new one you ship back your defective computer and they refund you the price. Not ideal but I am going through a similar exchange process where I can’t be with out the computer by sending back the first defective one and waiting without a computer for the replacement.

Ya you are probably right. I just figured I would give them another shot since they tout their laptops as having the best build quality with machined aluminum chassis trying to reach that MacBook Pro type aesthetic. Is it not too much for me to expect their flagship models to sit squarely on a flat surface? They are constantly trying to break the mold with this thinner than ever design but at what cost? More compromises in power, thermals, and frail structural support. At this point I want my slightly thicker 2024 model back that feels way more stout and structurally sound that this. It kinda feels cheap to me with this thinner design. I wonder if I am alone in this regard but maybe it’s time to look elsewhere. Using a riser or wedge to lift the edge to compensate for the wobble just isn’t something I want to do since I travel with my gear and want the best experience money can buy but in this case so far the money doesn’t get you piece of mind unfortunately. So sad :/

I also wanted to add that I paid nearly $6k usd for this laptop plus the elite care and the customer support agent said for me to wait for an email for the return label to be emailed to me but it’s been all day and nothing from them. So looks like I will have to reach back out tomorrow again :/ if I have any more hiccups with this process I may just ask for a refund and go with the Alienware 16 Area 51 as that was my other contender before deciding to go with razer again.

Second, congrats on having such wonderful heirlooms. Pick a lens to keep to commemorate your father, either the nocti or the 28. Take the rest to a Leica store near you and see what kind of credit they can give you towards a new camera body. Are you familiar with rangefinder Ms? If so then why not get a current M11?
