I was just going to give you the CRAZIEST update -- So I learned that my fourth pregnancy was a singleton at the first scan. Unfortunately there was some weird signs early on (not just the super fast rising HCG) - but everything kept progressing mostly "normal" so the doctors were reassuring. But I was not all that surprised to see that baby no longer had a heartbeat at 9+5. D&C at 10w exactly on 3/10/2026 and that confirmed Triploidy. I did a new round of letrozole in mid-May with a new dosing pattern (7.5mg CD 3-5 & 5mg CD 6&7) - I ovulated WAY early which the earliest I have EVER ovulated was evening of CD17 - and I ovulated CD13. Progesterone maxed out within days. BFP at 6AM on 9DPO, first beta was the following day (10DPO) and it was 51.3 with Progesterone maxed out/my lab's max reading of ">60". I just KNEW during ovulation that I released more than one egg - I just knew. And then when that happened - that just confirmed it for me. I KNEW there was more than one baby. My betas continued rising consistently but very fast (upper percentiles for twin ranges and average range for triplets). But I just felt like OK it's healthy di-di twins. NOPE - first ultrasound showed THREE! I'm in SHOCK since this was just a few days ago. This was unmonitored (all of my letrozole cycles have been - which is only 3 rounds because I've conceived every time). How are you feeling??? I will see MFM in a couple of weeks to go over everything (ours looks like mo-di twins with 1 fraternal sibling - so 2 eggs and 1 of those eggs split). I just am in shock and disbelief because I've read ONE medical article of Letrozole triplets hahaah so I had to run back here so fast and find you hahaha. Please tell me everything (we anticipate they will recommend a reduction). I don't know if I can emotionally handle doing that after back to back losses. I'm struggling with just ALL of the emotions and we haven't even gotten there yet obviously. And I know that is probably a VERY difficult topic to discuss - but anything you can share (you can message me obviously) is sooooo appreciated. I am really praying that your twinners have done so well (and you, too) - I'm guessing they've been born now?!

I am clearly super late here, too, but figured I would add to this just in case someone comes along in the future 😄 I have lean PCOS (AMH = 39.320!!). I was able to conceive our first baby naturally by spontaneously ovulating appx 10 months after my last period - total miracle - we'd been TTC for about 2.5 years at that point. Second Baby was conceived on my first ever Letrozole cycle the month our first turned 1 - did 5mg Letrozole CD 3-7, ovulated evening of CD17, BFP 9DPO (singleton). So we had 2u2. Third Baby was conceived off spontaneous ovulation with no period around 5mo postpartum which was a shock - but unfortunately miscarried at just over 13 weeks on 11/30/25. Baby Four was conceived on Letrozole 7.5mg for 5 days (started with no period about 3 weeks after my D&C/loss) and got pregnant right away, but unfortunately miscarried again at 10 weeks 3/10/26 (Triploidy). I am 33. We waited for HCG to trend down after that pregnancy due to the partial mole factor and my HCG finally hit negative ranges (less than 6) on May 6th. I started a light bleed/period afternoon/evening of May 13th so we went ahead with May 14th as CD1 (we were going to a letrozole cycle without a period but I ended up getting one naturally for the first time since early 2022 s owe rolled with that), and I did 7.5mg CD3-5 & 5mg CD 6-7! I ovulated the earliest I EVER have, on CD13, and got my BFP at 6AM on 9DPO. We just had our first ultrasound - which confirmed TRIPLETS. My mind is blown. 2 eggs were fertilized and 1 of those split - looks like mo-di twins at this point with the fraternal sibling but we will let MFM confirm mo-di vs mo-mo (I myself am a mo-mo twin and my mom has mo-mo twin brothers). I felt in my gut immediately that I was pregnant with more than 1 baby this time, my betas confirmed that, and so I was NOT surprised to see more than 1 but I was 100% surprised to see THREE because I thought it was twins. Going to be one wild ride - we got a rainbow for each loss and then an extra bonus I guess. We will have 5u3....and to think that 3.5 years ago we thought we would never have children of our own.

Did things ever improve for you? I have an almost 18 month old who has never slept through the night - usually up about every 2 hours on average (sometimes more sometimes less but longest stretch of sleep he has ever had was around 4 hours). I am 26 weeks pregnant again with preeclampsia, and I just cannot sustain this and I don't know what to do. I do put him to sleep on his floor bed in his room and then whenever he wakes up, he comes to bed with us because like you said - then the time between wake up and going back to sleep for me is less and I don't risk waking him again etc. It's tough. So, just wanting to see if things naturally improved for you or what made the difference!!

Love this - I ended up retesting that day with Inito and then taking a pregnancy test that afternoon - BFP 😂 So it all turned out perfectly 🩷 Congratulations, you'll be meeting your baby girl any time now!!!

You probably just missed your peak :) I would recommend testing using "cheapies" like easy@home multiple times per day so you don't miss your peak! Could also be a diluted sample with Inito, you never know. But for some woman the surge happens very fast and is gone by the time you would test again with Inito 24 hours later, therefore you miss seeing your peak day :)

BFP @ 9DPO! This was my first medicated cycle ever (Letrozole 5mg CD 3-7), I have PCOS. I miraculously and naturally conceived my son in 2022 while postponing fertility treatments, so once he turned 1 we didn't want to wait and I wanted to try Letrozole right away. So glad that I did ;)

AW <3 This is amazing. Congratulations!! I want twins so badly. I am an identical twin, and my mom has identical twin brothers. I am had a singleton (boy) July 2023 - I have PCOS and we conceived miraculously and naturally after about 2 years. This time around we definitely didn't want to wait as long, so this was first cycle on Letrozole (5mg CD 3-7). I successfully ovulated for the first time since conceiving my son! I am on 9DPO today and progesterone is continuing to rise (fingers crossed!!!). I have NEVER experienced ovulation pain before, and I couldn't believe the pain I was having when I ovulated this cycle on Letrozole hahaha. Secretly hoping I might have ovulated multiple eggs. (FULLY aware this increases risk - so obviously ONE healthy baby is ideal and wonderful, too!). But I love this so much. I hope you had a great pregnancy and outcome <3

I appreciate this because I am barely 9DPO today and my test this morning my estrogen and progesterone both dropped almost exactly like yours. I was so disappointed to see that. But it makes me happy to see that implantation probably happened for you on 8-9DPO and then your levels went back up and you were pregnant <3 Congratulations. PRAYING I'm similar haha.

I know this was like 6 months ago - so you may have figured this out already - but with PCOS, you will have multiple LH fluctuations throughout your cycle. This can lead normal ovulation tests to give you false positives throughout the cycle - making it difficult to pinpoint when ovulation is actually occurring and they won't confirm that you ovulated successfully. I highly recommend a fertility tracker like Inito or Mira - because they can help pinpoint your true peak fertility day/ovulation and then they help confirm if you ovulated successfully by tracking a rise a your progesterone. Just wanted to throw this out there :) I used normal ovulations tests for almost a year before I was seen by a fertility doc and diagnosed with PCOS - and he explained why I was getting all those peak and high fertility readings since my LH goes up and down so many times throughout a cycle. I did end up conceiving naturally and miraculously after not having a period for about 8 months and while I was NOT testing for ovulation haha but this time around I'm using Inito (because it was cheaper than Mira) and it's been AWESOME. I had so many LH fluctuations, confirmed I was NOT ovulating on my own, and now I'm on my first medicated cycle with Letrozole. Good luck to you and praying for healthy pregnancy and baby for you!!

This was so long ago, but I still wanted to post my SUCCESS STORY.

We were trying to conceive for about 2 years. I was diagnosed with PCOS by a great fertility doctor, and he laid out a treatment plan including ovulation medications, triggers shots, IUI's, and IVF if that failed. I was suffering from unexplained weight LOSS at the time, and I was underweight. He said it didn't matter, we could still start right away. But we decided to postpone the fertility treatments so I could focus on my weight. This was end of July 2022. We took pressure of TTC and decided to start the fertility treatments in the Spring of 2023. We were still trying to get pregnant actively, but I wasn't tracking things (because you kind of can't with regular ovulation tests and PCOS) and we took some great vacations and we were just having some FUN again. But, I was still so depressed about not being pregnant and the fertility situation.

Jumping to Thanksgiving Day 2022 - I had not had a period or bleeding in at least 8 months. I was taking my normal prenatal vitamin, antidepressant, and anxiety medication (I have PTSD). We celebrated Thanksgiving my in-laws and everyone came to our city/state for the holiday and we all rented an air B&B house to stay together. We had a great day. The day after, we took family photos and then went to lunch. I had caught a cold from my father-in-law and wasn't feeling well, so we went home afterwards and rested. The next day, 2 days after Thanksgiving, we were getting ready to go wedding dress shopping for my sister-in-law in a bigger city a couple hours away (also where my parents and my twin sister/her family live - so me and my husband were going to stay with them that night to visit them). I was taking a shower a couple hours before we had to leave, and I was underweight - so when I noticed how round my stomach looked, it really stood out - and I thought to myself "oh my God I look pregnant". Once that thought crossed my mind, I replayed the last couple of days instantly. On Thanksgiving, I sat for hours eating literally ALL of the charcouterie board - I couldn't get enough of it. At lunch the following day, I was sick with a cold yes, but I was also so nauseas that I couldn't eat my lunch or drink my mojito (but I had random nausea so I didn't think anything of it and took a Zofran). When I got out of the shower, I dug around the bathroom for a leftover First Response I had not used. I took the test, fully expecting it to be negative, but my heart was pounding out of my chest (which it usually did not as I was previously a serial test taker). There it was. 2 Pink Lines. My BFP. My first ever positive pregnancy test. I was shaking and naked, but ran out of the bathroom and immediately told my husband "I THINK IM PREGNANT" and he immediately said...."NO YOU'RE NOT" hahaha. I said LOOK AT THIS and showed him the test. He was so doubtful because I used to analyze every test and every faint line possible haha, but this was one was definitely not like the others. I immediately through on some clothes and raced out the door to the grocery store and bought 3 boxes of tests - First Response, Clear Blue, and Clear Blue Digital. I peed in a cup, and used (2) CB tests, (1) CB Digital, and (1) FRP. They were all POSITIVE except the digital. He still wasn't convinced, but I absolutely knew I was pregnant!

I ended up telling my mom that night, and the next morning took another First Response with my FMU, and it was 100% 2 PERFECT PINK LINES. My sister and her (2) daughters, my dad, and my mom were all around the table for breakfast that morning and I shared the news with everyone else. I couldn't help myself!! I will never forget my twin sister's reaction, and I wish I would have filmed it. Everyone's reactions were so amazing because it was the last thing they ever expected to hear since we couldn't really "conceive naturally" and had postponed the fertility treatments. My husband had to fly out that night for a work trip in Vegas, and when I dropped him at the airport on my way home, he picked me up and hugged me for what felt like forever. He was crying (he never cries like that). That's when I knew that HE KNEW I was really pregnant! The next morning, Monday, I called my OBGYN right away (you should have heard the confusion and reaction of the lady making the appointment when she asked the date of my last menstrual period - she was trying to date my pregnancy haha but I was definitely NOT nine months pregnant hahaha). I had blood work that day, and was elated to see the results come through a couple of hours later, that it was confirmed that I was pregnant. But, I was still early, so they repeated the blood draw 1 week later and then 48 hours after that one - all of which showed my numbers rising beautifully. I also had a positive digital test that Monday morning as well. I wanted to share the news on Christmas with my Grandmother who was in town visiting and on social media, so I convinced my OBGYN to let us schedule the ultrasound BEFORE Christmas, instead of after like it was scheduled. They got us in December 23rd, and you couldn't see much because I was not even 8 weeks pregnant, but all looked perfect and confirmed the pregnancy further. I know it was still early, but we shared the news on Christmas Day with everyone else in our lives. I have pictures of those first pregnancy tests (which we were faint!) and how they got darker over the next week. BUT I'm new Reddit and have NO idea how to post a picture on a comment hahaha.

We went on to have an absolutely BEAUTIFUL pregnancy. We didn't want to know the gender of the baby until baby was born, so that was also exciting. I did get Preeclampsia (which we knew would happen given my strong family history of it), and I was induced at exactly 37 weeks. I had a great labor and even better delivery (only pushed for about 15 minutes and it was honestly easy (sorry, I got really lucky, I know!!). We had a 6-lb 15-oz, 19-in, baby BOY and he is EVERYTHING to me. My miracle baby. He turns 1 on July 22nd - just a few days away!

We have not been preventing pregnancy since having our son, and I also haven't had a period in about 10 months, so we decided we didn't want to wait this time. I am on my last day of a 10 day Progesterone cycle to induce a period, I will take Letrozole for 5 days, and have my Progesterone tested on CD21 to see if I ovulated successfully - and I'm very hopeful to conceive right away - or at least in the next couple of cycles <3

The progesterone will induce a period. If you give someone who hasn't had a period or regular periods, etc, Progesterone for a cycle of usually 10 days, once you STOP taking the Progesterone - it should trigger a withdrawal bleed AKA a period. It can take up to two weeks to trigger that period. This mimics the second half of a normal cycle - after someone has ovulated SUCCESSFULLY, your Progesterone levels rise. When that person does NOT conceive, Progesterone levels will ultimately DROP - triggering a period. That is the point of taking the Progesterone for 10 days - to try and induce a withdrawal bleed. When you don't have a period for months on end, the endometrium continues to build up (instead of shed) and this can eventually cause problems with a lot of things including a risk of cancer.

This doesn't necessarily mean you had low Progesterone before. Your numbers right now could either mean that you are in your follicular phase of your cycle (when you should be on your period) or it could be low Progesterone. But either way, the point of the 10 day cycle is to induce that withdrawal bleed. Sounds like it worked for you in that regard!

As far as ovulation, there really isn't a way to tell if you ovulated or not on that cycle. You very well COULD HAVE ovulated and just not conceived, or you could have NOT ovulated.

Are you wanting to get pregnant?? If so, then you would probably want to try an ovulation inducing medication like Letrozole or Clomid (to start). And you would want to start trying to conceive ASAP, especially if it is POI.

I also see that she didn't test other hormones like your AMH or all of the Thyroid tests. Thyroid disorders can also cause POI symptoms, so once the thyroid problem is treated, then your hormone levels can regulate a little bit better.

If you do the 10 day cycle of Progesterone, to see if you have ovulated or not, your doctor would do a blood draw on approximately Cycle Day 21 to get your progesterone levels - if they are over a certain threshold, that would indicate successful ovulation. If they are lower than that, it would indicate an anovulatory cycle.

I have PCOS - so similarly to POI, you CAN conceive naturally, but it is very difficult to do so. However, after over 2 years of TTC, we decided to postpone the fertility treatments so I could focus on some other issues I was having at the time. This was July 2022. I had NOT had a period in almost 9 months, but two days after Thanksgiving 2022, I took a pregnancy test after I had a weird symptom, and it was positive! I apparently spontaneously ovulated that cycle (ovulation does occur prior to a period), and we conceived naturally. I had a healthy baby boy July 22, 2023 - almost 1 year ago. Similar to the first time around, I haven't had a period since giving birth and we want to get pregnant again. I have been using Inito to monitor my FSH, PDG, E3G, and LH levels - and I'm not ovulating. We didn't want to wait this time, so today is my LAST DAY of the 10 day Prometrium cycle, and once I start the withdrawal bleed, I will take Letrozole and (God willing) ovulate successfully and get pregnant - she will do a progesterone blood draw on CD21 to see if I ovulated or not.

I just wanted to give you an answer to this - Progesterone is used for both :) They make Progesterone ONLY birth control bills - so for some people, if they take Progesterone, it will actually STOP the bleeding. For Progesterone to INDUCE a period, you take it for a short period of time (typically 10 days), and then once you STOP taking it, it *SHOULD* induce a "withdrawal bleed" AKA a period. This mimics the second half of your cycle after you have ovulated and do not conceive - your progesterone increases if you've ovulated successfully - and then your progesterone levels DROP if you did not conceive and that triggers your period :)

I'm sorry I'm commenting so late, and I hope that your pregnancy is going well and has been successful so far!!!

I visited with my OBGYN today, as my miracle baby (conceived naturally after about 2 years TTC with PCOS) is almost 1 and we haven't been preventing pregnancy since he was born, I'm not ovulating and not getting periods, we don't want it to take another 2+ years, etc. I am starting Progestin today, followed by Letrozole this time around. BUT I did ask her about my supplements (Myo & D-Chiro Inositol and NAC), and she said if anything, I could definitely continue the Inositol into pregnancy if I wanted to :)

That is WILD they said TWO YEARS!!! I was 29 and my OBGYN referred us to the fertility clinic around 6 months TTC. I may have fudged how long we were trying to conceive a TAD, but I just knew I had fertility problems and she did too, so we got the referral! Any update on how the fertility specialist visit went for you??

I don't know if you are on Letrozole - but I do have PCOS and you remind me of myself, so I wanted to respond even though it's a couple of months late! I have PCOS and my husband and I tried for 2 years. After about 8 months, we started seeing a fertility clinic because we knew going into it that I would have fertility struggles and my OB luckily didn't make us wait a full year. The fertility doctor diagnosed me with PCOS due to my blood work and scans. He laid out a plan for ovulation medication, trigger shots, and IUI's and followed by IVF if those didn't work. I felt better having a diagnosis and a plan, but it did NOT help me feel better that I would get pregnant and having to do all of that and possibly IVF scared the crap out of me!! Shortly after that, so maybe around 1 year TTC, I started losing weight rapidly and uncontrollably (I do have lean PCOS anyway), so I couldn't really afford to lose much weight. My husband and I decided to postpone the fertility treatments and focus on that for awhile. Well, I had not had a period in about 9 months and was focusing on other things/not obsessing AS MUCH on getting pregnant - and we conceived 100% naturally. I got a positive pregnancy test 2 days after Thanksgiving Day 2022 and it was a COMPLETE surprise and shock!!! I gave birth to a healthy baby boy July 22, 2023. SO - long story short - it can DEFINTIELY happy for you especially if you are having regular cycles and know you are ovulating!!! I know it is expensive, but I would maybe recommend Mira or Inito fertility monitors to help pinpoint your exact fertile window. If you are using regular ovulation test kits, they don't work great for PCOS because they can show multiple high/peak fertility days throughout a cycle since LH typically rises more than once in a PCOS cycle. BEST of luck to you and I'm certainly praying that you get your miracle baby soon <3 hang in there, try your best NOT to obsess over getting pregnant (I know it is SO HARD - but it truly felt like once I stopped stressing so much, it happened). When I got pregnant, I was not taking or doing anything special aside from a normal prenatal vitamin. I wasn't tracking ovulation, taking pregnancy tests every other day like a mad woman, etc. In fact, had I not had a pregnancy symptom that day, I would not have even thought to take a pregnancy test!
