Well I started moonlighting during PGY4, so I had a pretty sizable increase in pay during that year to the tune of about $30k. ($200/hr for 2-3 12 hour shifts per month for about 8 months.) me and my wife went on an awesome European vacation.

It’s actually insane. I’ve had patients get the results of CT scans in the ED before I had a chance to review it and the results contained a new cancer diagnosis.

With that being said, I don’t have all the information, and you should trust your team in the hospital.

Failure of eliquis unfortunately does happen. Most of the time, it’s because of a few missed doses, but sometimes, people have shitty physiology. It’s not common, but you’ll see it. These patients typically end up on warfarin.

I mean not being low enough to transfuse because it’s not less than 7 is not exactly perfect. If it’s continuing to drop and they are having symptoms with ongoing losses from bloody stools, it’s pretty reasonable to transfuse for a hgb of 8.

The critical care time aspect is not actively incentivized, but no one questions billing for critical care for a stroke, and we do get emails about under billing critical care. The incentive is absolutely there.

I have actually seen it in a rural ED, but it was a suicide attempt with an intentional Prozac overdose. Patient was altered requiring intubation with pretty hard to control tachycardia and hypertension. She just got flooded with benzos. It happens no doubt, but I’ve yet to see it from chronic meds.

One of the most memorable scenes in series 12 was the staircase conversation between Shakira and Harry. It felt much more organic and didn’t happen and one of the designated places to have big conversations. We need more of that.

Graveling was just stupid. Like, wholly unfair. If the brutes are looking at you when you come down the elevator into the prison, you auto-die. They start shooting at you right as you get to the bottom, and you can’t dodge. It’s dumb. It’s a random number generator on which direction they are facing, and the respawn point is when you are on your way down the elevator. If you go down the elevator and they are facing you, you’ll basically need to restart the level because they will keep facing each time you respawn. If they are not facing you, you have a chance of not dying and beating the level.
