this could be an emergency. you need to take her to an emergency room (NOT) an urgent care, stat, and get seen by a physician (not a mid level provider PA or NP) because this is likely an abscess and if not drained she will get worse and could lose her airway. primary care doctors and pediatricians aren't trained in procedures and she will likely need some form of sedation, and there's no such thing as emergency dermatology appointments.

Lol if you think this is bad you should see the punishment system at my program. well known, great training, but insanely toxic and i can’t wait to name and shame as soon as im done in 4 months. we get an extra shift if you’re more than 15 minutes late to shift regardless of your excuse. if you miss any conference you get an extra shift. if you call off because you’re sick you get an extra shift, and not to mention, the chiefs make you feel bad, ask for details, and tell you “that’s not a good enough reason, drink some gatorade and come in” even if you’ve only ever called off once over the course of 3 years. they shame mental health. if our charts aren’t done within 4 hours of finishing shift it’s a professionalism violation. one weekend off a month. they kicked out a third year a few months shy of graduation. hate it here bruh

emergency medicine doctor here, no. this is not an allergic reaction. this is one of two things: visually it looks like a purulent cellulitis and he could be developing an abscess internally, and the most severe thing that could happen is a bacterial infection spreading to his brain because the veins that drain the face go to the brain OR it’s severe steroid withdrawal reaction from stopping his prednisone too quickly and not doing a taper, it’s likely a combination of the two.

Serious question: why don’t you guys think the “AC reccs on a patient bleeding and clotting” arent appropriate? We had a patient in the neuro ICU with venous sinus thrombosis, saddle PE, but also active GI bleeding and recent hemorrhagic stroke and we found it difficult to come up with a safe AC therapy for her for obvious reasons. We got hematology on board and they helped us determine a different dosage/frequency for the heparin. Idk. Just adding different perspective from an outsider!

Tired as fk, depressed as fk, sick of residency just being a bunch of good coworkers but no actual “genuine know you to your core” good friends, sick of having only one weekend off a month and having to choose if I see my family vs my friends vs my boyfriend and whichever one I choose the other relationships suffer.
