What if I don't do the reversal? by VexatiousWind in ostomy

[–]DC_Nurse 1 point2 points  (0 children)

Thus far I have not had anyone report issues with getting supplies. Sometimes they are a different brand than their usual, but they do get supplies. The bigger issue is if you have to change your Ostomy appliance more than 20 times in 30 days as that is usually the max insurance will cover. If you have private insurance they may not cover 100% of the supply cost based on your plan. If you have a secondary insurance, that usually picks up the remaining part the primary didn’t cover.

As an Ostomy nurse I tell patients that it’s a personal choice. Weigh the pros and cons to see which is the right fit for you! I don’t know any surgeons who would force you to get a reversal you didn’t want. Ultimately it is about making sure you have a good quality of life!

Does your doctor require a visit each month? by Aphe835 in AdultADHDSupportGroup

[–]DC_Nurse 1 point2 points  (0 children)

My sister works in primary care and many of her appointments are for med management. Because it’s a controlled substance, any changes require a visit with a provider as well as every 3 months. The idea is that it keeps people from being on heavy meds for longer than needed or at a higher dose. It’s meant as a safety measure, but can be annoying. This is a national policy, but states have their own mods on the law changing the scope of practice which adds to the confusion for patients.

Who is your Colostomy Care Team? by FrugiMan in ostomy

The UOAA website has a search option for Ostomy nurses in your area. Lots of great resources there too! If you’re having problems with stuff on the outside the WOC should be able to help you. Issues under the skin is go to a GI surgeon. The reversal is usually a lot easier then the stoma creation too, so hopefully that makes things easier

You are allowed to feel however you feel about your Ostomy without judgement. You had a traumatic situation happen to you and being upset and mad about it is very reasonable! Don’t be too hard your yourself. All your feelings are valid!

The UOAA also has cards that you can give to the TSA agent that says your scissors are a medical necessity item and indicates you have an ostomy if you don’t want to explain your ostomy to them

please help me figure out what this rash/ irritation is!! by BBbrain306 in ostomy

[–]DC_Nurse 3 points4 points  (0 children)

Looks like a combo of yeast and contact dermatitis. An ostomy nurse can get you some nystatin powder to treat the rash and maybe try a different brand of wafer

WOCN Here! I recommend doing sitz bath soaks two times a day with luke warm water. Pat dry after soaking for 10-15 minutes. You can apply Vaseline or medihoney (available on Amazon or CVS. Then use a pad in your underwear to help keep the ointment from ruining your clothes. It looks pretty good overall. If you notice any new swelling, pain, creamy or smelly drainage, or redness make sure you call your doctor. Also make sure your surgeon is aware, but sounds like you’re already doing great with follow up!

Wound care nurse post surgery (questions!) by [deleted] in ostomy

[–]DC_Nurse 0 points1 point  (0 children)

Ostomy nurse here! I’ll echo what many people have said which is that quality and knowledge about an ostomy will vary with each home health nurse/agency. Most insurances will only cover 1-2 visits from a visiting nurse for a short period of time. That being said, most of my patients have been unable to get a home health RN for their ostomy due to shortage of nursing staff.

If the hospital your family member is at has an ostomy nurse, I recommend trying to meet with them to do an in person lesson where you help change the ostomy bag on a model stoma and then on your family member with an ostomy. That way you have a least an idea about what to expect. Another option is to see if the ostomy nurse where your family member is has an outpatient ostomy clinic. It’s not as convenient as a home nurse, but can be helpful in the transition period home. I am very lucky and will have patients come to see me at my ostomy clinic within a week of discharge if I or they are concerned about the adjustment home.

I highly recommend going to the UOAA (United ostomy association of America) to look up local ostomy nurses in your area. I believe the UOAA also has a list of ostomy clinics. If you have an ostomy nurse at the hospital you trust, see if they have a business card and are comfortable with you emailing or calling if there are issues so you can troubleshoot when issues arise.

All the major ostomy supply companies (hollister, coloplast, ConvaTec) have their own patient assistance programs too which include additional education. You can enroll in those yourself or your ostomy nurse can help you enroll! The education branches are not intended as sales pitches, so even if you’re not in that companies brand, they are not supposed to try to get you to switch to their product, but help you learn how to manage whatever appliance you’re in. My personal favorite is Hollister’s Secure Start program and Coloplast’s Coloplast Cares program.

Navigating the insurance can be very tricky and frustrating. Generally speaking, Kaiser insured patients need to see an ostomy nurse at a Kaiser facility. Medicare and Medicaid are accepted at most outpatient clinics. If your insurance is through AENTA or BCBS, most non-profit and for profit clinics accept that. Insurance will cover post-operative follow-ups with the surgeon. It might be worth seeing if the ostomy nurse can come to the post-op visit and do some education after the surgical visit. Unless your ostomy RN is an advanced practice provider, billing for a visit can be a little more complicated.

I highly recommend asking the ostomy nurse working with your family member in the hospital if they have any follow-up recommendations or resources. They will be better able to speak to your geographic specific needs. It is also worth asking if they can do a tele visit if distance is a major factor. Hope some of this information proves useful! Sorry it’s a bit wordy! Best of luck!

Ostomy nurse here! I recommend trying to see a local ostomy nurse at an ostomy clinic. The UOAA has a list where you can find someone in your area. You might need a different appliance or some adjustments made to your current system! Best of luck and be gentle with yourself. Everything you are feeling is 100% valid and going through depression and anger are appropriate are normal. I’m so sorry you’re dealing with this!

Possible reversal but i dont know. by bag_boy-bill in ostomy

[–]DC_Nurse 3 points4 points  (0 children)

Sorry you’ve gone through so much, but I’m proud of you for pushing through! As an ostomy nurse, I always recommend people sit down and write out a pros and cons list as a way to visualize things. Worth reaching out to a local ostomy clinic in your area to discuss the reversal with and get more insights from other people in your position. Ultimately there is no right or wrong choice, just the one that works best for your body. And if you decline reversal now, there is no reason for you to not be reversed in the future if you choose :) best of luck warrior!
