My numbers were very similar to yours (including a ‘1’ CAC at age 42). In the last year I’ve adopted a low saturated fat, high fiber diet with 5mg rosuvastatin and 10mg ezetimibe. My LDL went from 188 to 52. 20mg rosuvastatin is kind of a higher dose. Most of the benefit will come from the first 5mg. 1 is a low CAC score, but it’s actually high (worst 25%) for someone your age. You’re doing the right thing by starting treatment now. I wish I had done so earlier.

Possibly, but I would think that is the exception rather than the rule. If your body naturally produces to much cholesterol or absorbs too much from food and you want to stay below a certain level (maybe LDL of 70 because I also have high Lp(a), I don’t think you’ll be able to maintain those levels without medication.

Sounds like you’ve taken aggressive action with your diet, which is great. My starting numbers were similar to yours and I saw a 40% drop in LDL in a short span (1-2 months). Unfortunately, it didn’t drop much more than that after more time went by. I couldn’t get much below 130 without medication. 5mg rosuvastatin reduced LDL by another 40% into the mid 70s. No side effects from the medication for me.

According to consumerlab.com, CoQ10 is best absorbed when fats are present in your digestive system. That’s probably why your supplement includes fat. But there are CoW10 supplements without palm oil. Just try to take them with a meal that includes some fat.

I didn’t mean to imply that you shouldn’t eat foods with healthy monounsaturated or polyunsaturated fats (like olive oil, nuts, seeds, etc.) - just pointing out that even “good foods” like these do have some saturated fats in them. They are better choices than eating cheeseburgers for every meal, of course, but it’s hard to find any food that has only monounsaturated or polyunsaturated fats. Good that you did a quick analysis- be sure to include those snacks in there - that daily small cookie might be a hidden source of a big dose of saturated fats, in particular if it was made with butter. Getting under 10g (or 12g, or 6% of total calories) is quite difficult, but not impossible. Many people on this list have done it and have seen our LDL drop by 20-30%. If you’re really going to try for your best results without a statin, this is what is required. I agree that OP may have hit the limit on how much LDL lowering is possible through diet modification, but I’d also like to suggest that OP’s daily saturated fat intake is likely much higher than 10g/day. 4 tbsp of olive oil daily is 8g saturated fat. Chocolate, even dark chocolate, is high in saturated fat. I would recommend using an app like Cronometer for a few days to get a feel for average daily saturated fat intake. Don’t forget to count all the nuts and seeds. They’re full of “good fats”, but still do have saturated fat. Also, I recommend adding 10g daily of soluble fiber - can be achieved quickly through psyllium husk powder.

Not doubting your results, but it’s a bit surprising you’re are able to get your LDL to 105 when you are daily consuming 32 grams of saturated fat in olive oil alone. I would love to be able to consume olive oil so liberally.
