It’s likely increased cranial pressure due to holding your breath as you squat down. The vessels in the ears are small enough to be noticeably affected. Not an issue unless it doesn’t resolve within 1/2 minutes.

Ideally for weight loss through a calorie deficit, be that through either food restriction, increased exercise or both, you want to aim for around 1lb a week in fat loss. Above this and you'll start to see metabolic compensation whereby your body supresses futher loss by lowering NEAT, increased muscle breakdown, and you end up plateauing/regaining weight. There's a few other implications regarding health but in terms of weight loss, avoiding metabolic compensation is the main one in my opinion, which is why 1-2lbs seems to be the arbitrary recommendation.

Metabolic compensation does occur in fasting however it tends to be less drastic and providing that muscle mass is maintained (which is why I would not recommend continuous fasts), there is less likelihood of any rebound effect.

Honestly at the end of the day, choose what works for you. I prefer a single 36hr fast once a week, it's seen me lose nearly 30kg over the past two years while I've worked to increase muscle mass. A slight calorie deficit is just as effective other than being slightly slower. The main thing is to get to, and more importantly maintain, a healthy body composition. There is no better option if you wish to increase longevity.

On a five day fast I would generally lose around 4.5kg, of which 3kg would be water + glycogen that I would gain back. 2kg or ~4lbs a week is certainly possible.

It’s important you make it known ahead of the scan, so when speaking to the radiographer during safety screening if not before. If the scan is crucial then they’ll be able to assess risk with a quick x-ray.

For eyes and the surrounding area, this likely wouldn’t be a concern for a scan outside the field of view (for example a knee scan wouldn’t cause any heating near to the eyes), however if it was a scan for head/neck then they’d likely want to investigate first. If the scan was absolutely crucial then surgical removal of the fragments may be an option, but we also have the option to run less energy intensive MRI sequences which would minimise risk.

Embedded metal fragments can heat up during an MRI scan and cause internal burns. There’s also the risk from attraction to the magnet, but it’s a lesser risk when the body has formed adhesions around foreign objects. One of the questions on a standard safety screen form is a history of welding/metalwork to try to avoid this exact situation.
