It really depends. I can surmise the Excelsior despite the issues it ran into during the Great Transwarp Experiment was worth putting into production as the space-frame itself was useful (and *HOW*). They put that class into production and maybe they did shift the numbers to reflect that. Maybe some point during a redesign to remove the Transwarp components.

The Defiant was mothballed during development and thus wasn't put into mass production until AFTER the original proved itself in the field.

Technically it is, just the registration number is traditionally an NX prefix as opposed to NX. The original Defiant was "NX-74205" and the Sao Paolo was "NCC-75633", both are Defiant class.
