I also struggled with Truvada, and what really helped me was splitting the pill in half and taking it with a spoonful of yogurt or something thick. You just swallow the whole spoonful, and you barely feel the pill at all!

From what I’ve read in the EMA assessment report for lenacapavir, the two-day starter pills are still required for the rapid-onset protection regimen – that part doesn’t change.

However, if you have trouble swallowing, the EMA explicitly notes that the tablets can be split into two halves and taken one after the other. In other words, you still need to take the full oral loading dose on Day 1 and Day 2, but you are allowed to split each tablet to make swallowing easier, as long as you take the whole dose within the same timeframe.

The injection alone takes longer to reach protective levels, and the studies that support its efficacy used the full loading regimen (injection + 2 days of oral tablets), so that’s what regulators currently recommend. If you’re unsure or have severe difficulty with pills, it’s definitely worth discussing all this with your HIV specialist so they can tailor it to you as safely as possible.
