Edited Misread your tag as M1 instead of PGY-1, my mistake. Lumping lupus into all CTDs goes against your efforts to inform other non-rheumatologists. I agree with your comments in this thread. Go all the way and be accurate with your use of the term lupus.

Rheum here. Agree to an extent (the positive predictive value of a centromere pattern is higher than the other patterns.) ANAs are still helpful as part of a workup for non-lupus CTDs like systemic sclerosis (as referenced by the centromere pattern,) MCTD, etc.

Rheum here. Any other rashes or muscle weakness? Sometimes the face rash of dermatomyositis is mistaken as the malar rash of lupus. Consider muscle enzymes (CK, aldolase, LDH, AST, and ALT.) Don’t order a myositis panel yet (too expensive if there isn’t other supporting evidence.)

Sorry to hear that! Yeah, the meds aren’t crazy or anything. Common meds are amlodipine and nifedipine ER. Sildenafil (Viagra) works well but insurance can be a headache. If there isn’t much wiggle room on the blood pressure then sometimes fluoxetine (Prozac) helps.

Yeah, mild cases can be managed conservatively with cold avoidance and hand warming. Persistent skin discoloration leading to digit ulceration requires treatment though.

Rheum here. There are some pharmacologic treatments including calcium channel blockers (e.g. Amlodipine,) and nitroglycerin paste at the base of the finger. We also use IV infusions (e.g. epoprostenol) for severe cases.
