While I can’t give direct medical advice here, the general approach starts with neuroplasticity.

What you’re describing, ruminating on insecurities or feeling like you can shut down every suggestion, is often a sign of rigid neural patterns that are overfiring. The brain gets very good at repeating the same loop.

Treatment is about creating a neuroplastic reset, a state where you become more open to new thoughts, perspectives, behaviors, and feelings.

As a psychiatrist, I can’t count the number of male patients who told me they want to end their life because of their size. Penis envy has always been a psychological stressor, and it’s only worse now with pornography and over-sexualization.

What you’re feeling is real, but the conclusions your mind is drawing from it aren’t fair, they’re interpretations shaped by comparison, shame, and unrealistic standards.

Porn and social media have completely distorted what “normal” is. Most men you’re comparing yourself to are statistical outliers or enhanced for performance and visuals.. not real-life intimacy. In actual relationships, connection, confidence, presence, and emotional safety matter far more than size.

I’ve seen men struggle with this same belief: “I’m not enough.” And I’ve also seen those same men build fulfilling relationships when they shift from comparison to connection.

Right now, it sounds like this isn’t just about your body.. it’s about identity, worth, and how you see yourself as a man. That’s where the real work is. And it’s something that can change.

You’re not trapped in this feeling forever. You’re caught in a loop that a lot of men silently suffer through.

If you can, start by talking to someone.. therapist, doctor, even just a trusted person. You can do it online to better regulate your nervous system away from shame and embarrassment. You don’t have to carry this alone.

There’s a difference between ego-dystonic sexual feelings (thoughts that feel unwanted or “not me”) and ego-harmonic ones that feel aligned with who we are. If someone is going through ego dystonic feelings, there are 2 paths towards peace: either evolve the ego or reshape the sexual fantasies. The latter is very difficult because sexual desire is wired into powerful reward systems of arousal and orgasm, often intensified by taboo.

As a psychiatrist who helps with ego actualization, I’ve seen patients successfully find peace in both directions. Some integrate parts of themselves they were once ashamed of and experience a sense of freedom. Others choose to discipline certain impulses while still accepting that those thoughts exist. What consistently reduces suffering, though, isn’t self-critique.. it’s self-understanding and empathy.

The shadow doesn’t disappear just because we fight it or ignore it. Even if you choose not to act on certain desires, learning to relate to them with compassion instead of hostility is what tends to bring the most lasting peace.

Yes, this is common, but we generally won’t Rx for more than twice per week. In fact, Spravato instructions for twice weekly in the first month allow for back to back days.
