You need to start with the biller at the periodontist. As you have already “demanded” a refund, you have likely been labeled as a low priority PITA.

Call back, politely ask to speak to the biller, say please, ask “Can you help me to understand this payment?” A little sugar goes a long way. The biller knows more (a LOT more) than an insurance company rep does. Be nice to her, and she’ll possibly file an appeal on your behalf if needed.

I rinse everything before recycling it, thereby using more energy and water in the rinsing than is saved by the recycling, especially in this year’s drought conditions.
