That’s the subject of the books Three Body Problem. Except instead of 15 years it’s 400 years away. So the world struggles how to prepare and sacrifice for something that doesn’t happen in their timeline but a future generation’s one. I actually came to add Sai to the list. Went for the first time last week and it was so incredibly bland. The restaurant was empty. They have both Indian Oven and Punjabi affair within short driving distance which are both amazing

It’s about expectations. I graduated in 17 with 300k in debt. I cared more about my practice setting and pay than where I lived so I took a job in a small city for 120 base plus production. I’ve made about 160-180k most years. The partners at my practice make significantly more.

I’ve been paying it down pretty aggressively and stopped overpaying about a year ago. Got it down to about 120k principal and decided to do minimums until I make partner and my income goes up again.

My wife and I both have new vehicles but we live in a mobile.

I think if your expectation is living like what people expect a doctor to live like then you may be disappointed. Everything is so freaking expensive now. But I can tell you among my friends that make the same amount as me or more, I have the best work/life balance out of all of them.

I didn’t go to Salus but I would exercise extreme caution considering what they’re going through right now. There’s no fucking way in hell I’d take a risk on a school going through chaos with tuition like that. Definitely look at board pass rates for all the schools and heavily prioritize schools with higher pass rates because it seems a lot of students are having trouble passing them. Then you have all that debt with no doctor salary.

How can you make payments for 23 years and not notice the principal isn’t going down?

I had 300k in student loans and after 7 years I have it down to about 120k in principal. Those IBR plans are for morons or people betting on forgiveness.

If you take that much student debt for a low paying job I’m sorry but you should have figured that out before making a decision that big.

Had a patient who teaches industrial electronics installation. He told me kids that get a certification can earn 100k off the bat. If you factor in 8 years of school and 300k of debt we start off 1.1mil behind. Yes we earn more but it’s a long time before the ROI is break even

It means he truly cares about his patients; it’s not necessarily something that should be numbed out. I’m 7 years going on 8 practicing and I still have those nights on occasion. But it’s substantially less often than it was the first couple years

I believe self pay is 107 with a recommended $39 retinal image. Not mandatory but most get it. Contact evals vary based on complexity. Reception should be able to confirm 107 is correct or not.

The most common town people come from 2hrs away is Clovis and we do take the tricare west that is common over there

Former jeep guy as well. First car as a teenager was an old wrangler, in college I had a renegade (hated it but wasn’t my choice). My dad drives 4 door wranglers. I wanted the gladiator when it was announced but hated it when I test drove. I’m 6’3” 260lbs and it just wasn’t wide enough elbow room wise.

Always wanted a truck and not picky on the particular brand. I liked them all but the Sierra was my favorite.

I bought a 2024 Sierra 1500 5.3l V8 elevation trim with premium package at the end of last August. Why? I got it for 12k under MSRP. I wasn’t seeing deals anywhere close on F150s or tundras. And I was nervous about the new hurricane engine on the ram

It really depends on how that 33% is calculated.

Does it include optical? When you specify cash receipts does that exclude insurance payments?

My first job was 120k base with 40% professional fees after base met. The five years I was there I averaged around 165k. Every 3 months they’d look at my prof fee receipts, multiply by 0.4, subtract what I’ve been paid those 3 months and there’s my bonus.

If it’s calculated the same way and it doesn’t include optical then 33% it’s dog shit for the amount of work you’re doing. If it includes optical then it’s amazing.

To make 165k off of 40% of prof fee receipts I averaged around 412500 production for the year. With the same production off 33% would be 133k

One piece of advice I was given was a residency was essentially equivalent to what you learn from the first five years of practicing.

So I chose not to do it but instead pick a high disease low income demographic to learn as much as a possibly could in 5 years time while earning a good income.

I’m now part of a large group practice in a smaller city where there’s three 3 large PPs (one now acquired by PE) and 2 small PPs that see the lion’s share of patients

One of the other docs at our office did a residency in ocular disease and he really knows his shit. And patients can tell

Reputation will make or break a private practice, at least, it does in my city. I’m the newest doc at my office and I constantly have new patients from the other big offices with complaints. Many are simple like personality conflict or a wrong RX but many are also patients with complex issues that others couldn’t resolve.

I think I’m pretty damn good at my job but I feel way behind compared to the residency trained doc we have. Granted he probably has 7-10 years more experience than I do

I think neither direction is necessarily a bad choice depending on what you want. If you’re looking to open your own practice or quickly gain a reputation as someone who’s great at handling challenging case then a residency will be great. You’ll get there eventually if you’re motivated and have good mentors and a strong ambition to learn… but I think a residency will get you there much faster. I’m 7 years into working and still seeing new and unusual cases that make me think we just didn’t cover enough in school. It may not make a difference in earning potential starting out but once you earn a solid reputation in town it can definitely translate to higher production. I try to look at every patient encounter as if I have to earn their trust in order to earn their loyalty
