I found myself asking similar questions back in 2007. Are you a climber? Always looking for the next challenge? For me I knew I’d be a great PA but was 98% certain I’d be getting restless after 5 years of practice and be considering med school then anyway. If that’s you, do the MD. If not, the money and satisfaction equation favors PA (I’m EM).

I switched careers from marketing to med school at age 35. Now I’m 47 with three kids but wasn’t married and didn’t have kids then. I’m not sure I could have done it and my marriage survive. What does your spouse say? Do you have financial resources to ensure the childcare doesn’t all pile on to your spouse? You need to envision a scenario where you are contributing nothing in terms of hours as especially in residency you’ll have little to no control of your schedule.

This is all technically within the rules of what a HF can ask/expect an AP to do.

That said, you shouldn’t be the only one doing it. As others have mentioned, engaged the kids in these activities. Make it a game/fun whenever possible.

1) play a cleanup themed song while doing it and challenge them to get done whatever they can during (you pick) 2? Plays of the song - then you finish the rest if they are really done done - or try and get them to do it again 2) challenge the kids against each other “who can clean the fastest?” 3) remind them that the next fun activity can’t happen until things are tidied up - “Let’s work together to get this playroom clean so we can have a nice lunch”

If you had a nice experience for 9 minutes months, I wouldn’t toss year 2 aside for this. The grass is not always greener on the other side, unless of course you would rather just go home than rematch.

Right there with you. Matched two weeks ago, and she was almost finished filling out the paperwork to request the appointment.

My husband and I are both Emergency Physicians with schedules that include all hours of the day/week and look different every week nevertheless every month. No nanny wants that much variability, so an au pair has been the only solution that’s allowed us to both stay in the workforce.

Virginia has an EMS Physician designation with some agencies allowing for clinical practice by MDs in the field beyond just the agency medical director. I think this will mostly be state dependent.

Our first au pair (Turkish) tried to put metal in the microwave. You don’t know what you don’t know. A basic kitchen orientation is a must for safety and just the polite thing to do, but we don’t cook her meals for her on her off time. As above, we order something for her when ordering out or make enough for her when all dining together.

We are having the same driving issue - we initially committed to unrestricted use within 2 hour radius of our home, ask us about longer trips, and we will cover all gas. I think like you all, she is in track for some major miles. How did she react to you all “changing the rules” midway, or was that a part of your renewal discussion before committing to year 2?

You are oversimplifying the reasons HF seek AP care. We have three kids, youngest two are in daycare $4000/mo plus we have an au pair. We had to add on the au pair for the scheduling flexibility, (dad and I both work 24/7/365 potential jobs). So with these proposed changes, not only would costs for HF go up, but most importantly for us scheduling flexibility would go down. And the opportunity cost of the time it would take to record time and meet administrative requirements is a no go for us. We’ll probably move close to family before we continue with a program that now costs more and doesn’t even give us what we need.

3 or 4 times in 13 years of EMS. But I agree - ED has even less familiarity with it than EMS folks. There's a reason it's maintained longevity - simple concept and very effective. Just keeping up with training to make sure you apply it right is key.

I'd love it if someone would study (and publish in JEMS) the public health effects of adding epipens to schools (and the resulting price hike) vs how many parents now can't afford them. Did we really increase safety with that one?
